The Patient Care Coordinator (PCC) in the Emergency Department supports a safe, efficient, and patient-centered care environment by coordinating non-clinical aspects of care delivery and enhancing communication between patients, families, and the interdisciplinary care team. This role directly supports organizational goals related to patient experience, quality, safety, and regulatory compliance, including Press Ganey performance and Joint Commission standards.
The PCC serves as a visible and accessible point of contact for patients and families, proactively addressing concerns, setting expectations, and promoting clear, respectful communication throughout the emergency department visit. By facilitating patient flow activities such as admissions, transfers, and discharges, the PCC helps reduce delays in care and supports timely, coordinated treatment.
In alignment with Joint Commission requirements, the PCC assists with accurate documentation, patient identification processes, handoff communication, and escalation of safety concerns. The role collaborates closely with nursing leadership, physicians, case management, social work, and ancillary services to ensure care coordination, appropriate follow-up, and adherence to established policies and standards.
The Patient Care Coordinator contributes to a culture of safety, service excellence, and continuous improvement by identifying opportunities for process improvement, reinforcing service recovery practices, and supporting initiatives aimed at improving patient satisfaction, throughput, and quality outcomes in the emergency department.
