Sometimes the best healthcare is offered right in the patient’s home. In those cases, the home health or hospice aide becomes more than just a nurse. They become a patient’s lifeline, confidant and friend. Our home health aides go beyond taking vital signs and administering medicine. They also provide personal hygiene assistance, help with small things around the house, pray with our patients, and prepare light meals or snacks – all while accurately assessing the patient’s status and communicating it back to the supervising clinician.

WHAT YOU CAN EXPECT

Assists patients with care and activities of daily living as directed in the established Plan of Care.
Completes necessary documentation of care provided. Records intake, output, and vital signs as directed and communicates care as appropriate.
Provides assistance with personal hygiene and elimination needs according to the established plan of care.
Assists patients with ambulation and transfer using proper body mechanics.
Communicates any change in condition or lack of patient progress.
Schedules visits with the patient or caregiver.

QUALIFICATIONS

High School Diploma/GED - Required
1 year Home Healthcare - Preferred
1 year Long Term Care Setting - Preferred
Certified Nursing Assistant (CNA)- State of Illinois - Required
Cardiopulmonary Resuscitation (CPR) - Upon Hire- Required
Auto Insurance - Upon Hire- Required
Driver's License - Upon Hire- Required

TRAVEL IS REQUIRED:

Up to 75%

Home Health Aide $18.02-$23.43

INCENTIVE:

This position is eligible for shift pay for eligible shifts, and call pay for qualified on-call hours
