The Home Health Hospice Aide, under the supervision of a licensed clinician, participates in the delivery of patient care in the home or inpatient setting.

WHAT YOU CAN EXPECT

  • Assists patients with care and activities of daily living as directed in the established Plan of Care

  • Communicates any change in condition or lack of patient progress.

  • Completes necessary documentation of care provided. Records intake, output, and vital signs as directed and communicates care as appropriate.

  • Provides assistance with personal hygiene and elimination needs according to the established plan of care.

  • Schedules visits with the patient or caregiver.

  • Assists patients with ambulation and transfer using proper body mechanics.

QUALIFICATIONS

  • High School Diploma/GED - Required

  • 1 Year Long Term Care Setting Experience- Preferred

  • 1 Year Home Healthcare Experience - Preferred

  • 1 Year Hospice Experience - Preferred

  • Certified Nursing Assistant (CNA) OR Home Health Aide (HHA)- State Licensing Board - Required

  • Cardiopulmonary Resuscitation (CPR)- American Heart Association - Required

  • Auto Insurance - Required

  • Driver's License - Required
