RN Hospice On-Call - Nights

Accomac, Virginia Schedule: This position is four nights per week, 4:30 PM – 8:00 AM , Tuesday through Friday , with some flexibility as needed. Overview Works under the supervision of the Nurse manager and the Director and is responsible for the assessment, planning, implementing, and evaluating the total nursing care of assigned patients on an intermittent visiting basis. The RN may work in a variety of settings (typically in a home care type setting) and roles within Riverside Health System. The RN utilizes communication, critical thinking, problem solving, and decision making to create a care plan for each patient based on physiological, psychosocial and cultural diversity. The RN On-Call provides support to the agency after hours via phone or patient visit as necessary. Will take calls from patients regarding issues or concerns and triage the clients needs as to whether a visit is necessary or if the problem can be handled via patient/caregiver education via phone. Will be responsible for taking any after hours referrals and forwarding information to the Care Manager on call. Is responsible for assessing all of the patient's physical, functional, ADL, psychosocial, spiritual, communication, cultural and environmental needs when applicable via phone or via patient visit.
