Responsibilities Involves patient, family, and health care team members in formulating an appropriate individualized plan of care and identifies expected outcomes based on nursing assessment and diagnosis. Implements the plan of care in partnership with the patient, family, and health care team members. Ensures all components of the plan of care are documented in the patient health record.
