Regional MDS Coordinator 

Location: West Region (Carthage area with occasional travel to facilities as needed)

Position Summary:
We are seeking a dedicated and detail-oriented MDS Coordinator to oversee the Minimum Data Set (MDS) assessments and care planning processes for two skilled nursing facilities with small census populations. This is an excellent opportunity for an experienced professional to make a meaningful impact in ensuring regulatory compliance and delivering high-quality care.

Key Responsibilities:

  • Oversee the MDS process for the region, ensuring timely and accurate assessments.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans.
  • Monitor and ensure compliance with state and federal regulations.
  • Conduct audits and reviews to ensure the accuracy of submitted data.
  • Provide education and support to staff regarding MDS processes and regulatory updates.
  • Serve as a liaison between residents, families, and staff to ensure care plans meet resident needs.

Qualifications:

  • Active RN or LPN license in Missouri
  • Minimum of 2 years of experience as an MDS Coordinator in a skilled nursing or long-term care setting.
  • Knowledge of RAI process, state and federal regulations, and reimbursement systems.
  • Strong organizational and time management skills with the ability to manage multiple priorities across two locations.
  • Excellent communication and teamwork skills.
