Now Hiring: MDS Coordinator – Skilled Nursing Facility
We are currently seeking a dedicated and experienced MDS Coordinator to join our team at a Skilled Nursing Facility. This key leadership role is responsible for managing the resident assessment process, ensuring accurate documentation, coordinating care plans, and contributing to high-quality outcomes across the continuum of care.
Key Responsibilities:
- Lead the completion of timely and accurate MDS assessments in accordance with federal and state guidelines.
- Coordinate interdisciplinary team efforts to develop, review, and revise resident care plans.
- Ensure accurate documentation for proper reimbursement from all payor sources.
- Monitor care delivery to ensure resources are effectively allocated and quality care is consistently provided.
- Support survey readiness and compliance with regulatory standards.
Qualifications:
- Current and active nursing license (RN or LPN).
- Minimum 1–2 years of MDS experience in a long-term care setting.
- Solid knowledge of long-term care regulations, nursing practices, and MDS processes.
- Experience working in skilled nursing or long-term care environments is preferred.
Skills & Competencies:
- Communication: Able to clearly share information with interdisciplinary teams, residents, and families.
- Organization: Strong ability to manage multiple assessments, timelines, and documentation.
- Leadership: Skilled in leading care plan meetings and supporting nursing staff.
- Management: Capable of overseeing documentation, quality measures, and patient record audits.
