Summary:

Provides comprehensive nursing services to individuals and families in the community setting; Places emphasis on meeting the needs of persons in their homes, upon promotion, and maintenance of health, and on early detection and restoration measurers. 

Essential Duties and Responsibilities:

Conducts comprehensive OASIS initial nursing assessments, including physical, emotional, and mental needs

Determines eligibility for services related to homebound status and need for skilled services

Develops an individualized plan of care in consultation with the patient’s physician and other agency personnel as appropriate

Completes review of medications, pain management, risk status

Develops and implements the care plan for the patient and family

Provides intermittent skilled services, as ordered by the physician, and maintaining the highest quality of care for the patient in the home environment

Obtains specimens as ordered by physician, submits to laboratory

Participates in team meetings/patient care conferences, communicates with other clinicians involved in care

Provides direct nursing care procedures to patients; Updates primary physician when necessary and at least every 60 days

Completes OASIS at all points of care as required under PPS

Responsible for thorough and complete documentation to ensure continuity of care, professional services, and the maintenance of accurate legal documents

Teaches family members, and/or significant others, patient care skills, accurately assess patient’s physical and emotional status, and perform services in compliance with physician orders

Participates in on-call duties as defined in the On-Call Policy

Ensures that arrangements for equipment and other necessary items and services are available

Instructs, supervises, and evaluates home health aide care provided at least every two (2) weeks

Attends in-services and department meetings as required

Qualifications, Knowledge, Skills, and Abilities:

To perform the job successfully, an individual must be able to perform each essential duty satisfactorily.  The requirements listed below are representative of the knowledge, skill, and/or ability required. 

▪Current Missouri RN Nursing License in good standing

▪Graduate of an accredited School of Nursing

▪Must be CPR BLS (basic life support) certified with the American Red Cross

▪Prior home health experience preferred

▪A minimum of two years nursing experience preferred

▪Ability to provide own transportation

▪Has good clinical judgment, assessment, and communication skills

▪Proficient in computer skills, to complete documentation on laptop

EverTrue Pay & Benefits

Pay grade: 14, $31.05/hour to $49.75/hour. Eligible for shift differentials: Yes. $3.00/hour evenings; $2.50/hour nights; $2.50/hour weekends (States of IL & MO; May vary in other states)
