Responsibilities:

Utilize the nursing process to assess, plan, implement, and evaluate patient care.
On each assignment, provides skilled nursing care/services in accordance with prescribed orders
Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition.
Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.

Qualifications:

Currently licensed as an LPN/LVN in the state in which the LPN will practice.
Current TB or Chest X-Ray.
Current BLS card.
One year prior professional nursing experience preferred

What you will do Accurately completes all required clinical documentation consistently per Riverside Medical Group standards. Maintains documentation for quality assurance to include but not limited to point of care testing, medication logs and annual wellness visits. Works with providers to assure care gaps are routinely addressed with each patient. Documentation is accurate, legible and completed same day. Knowledgeable and skillful in lab functions and lab proficiency testing. Maintains excellent clinical competency by adhering to the standard clinical expectations for Riverside Medical Group. This includes updating, verifying, and maintaining patient records by properly interviewing the patient and confirming purpose of visit. Prepares patient for examination by obtaining vital signs proficiently (including manual vital signs when mechanical devices are not available).
