Responsibilities

What You’ll Do:

  • Administering medications and monitoring vital signs, ensuring accurate documentation in medical records.
  • Evaluating medical complaints and concerns from residents, effectively communicating with our Nurse Practitioners and Physicians.
  • Developing and implementing treatment plans, providing direct care and nursing services as prescribed.
  • Monitoring chronic health conditions, respiratory distress, and seizures, ensuring timely interventions.
  • Maintaining comprehensive medical records and logs, adhering to all mandated reporting and infection control protocols.
  • Collaborating with Care Coordinators regarding medication management and refills.
  • Participating in interdisciplinary treatment team meetings, contributing your expertise to enhance patient care.

Qualifications

 

    • Education: Graduate of an accredited nursing school with a Nursing Degree;
    • Licensure: Current LPN/LVN licensure with the California State Board of Nursing is a must.
    • Experience: Ideally, you bring previous experience in a healthcare setting, preferably with adolescents and/or young adults
    • Communication Skills: Exceptional verbal and written communication skills, with the ability to make sound judgment calls in high-pressure scenarios.

    **Pay range for this position is $26.95 to $37.73 depending on education and experience.

Job Title: Licensed Practical Nurse (LPN) Department: Long Terem Care Location: Clifton Springs Nursing Home - 2 Coulter Road Clifton Springs, NY 14432 Hours Per Week: Per Diem Schedule: Per Diem - As Needed Key Responsibilities: Responsible for timely, accurate medication administration of assigned team and concurrent required documentation. Performs treatments and documents progress of same as per physician orders and facility protocol. Assures concise, pertinent and complete resident care documentation using computerized medical record process. Assures the initiation of appropriate care pathway tools. Directs CNA assigned to the team in the timely completion of quality individualized personal care in compliance with resident's individual care plans. Ensures CNA documentation is accurate and complete. Responsible for receiving/giving relevant reports, both written and verbal from/to appropriate staff as per departmental protocols. Contacts physicians, transcribes orders and follows up with appropriate action to institute orders in timely manner. Assures proper monitoring of residents and relevant condition status are reported to charge nurse/supervisor and/or physicians in a timely manner. Completes pre-reviews of medication administration records as per facility protocols. Completes required MDS documentation and all related observations in a timely manner. Supports CNA team through assisting in direct ADL care of residents. Minimum Qualifications : Graduate from an accredited School of Practical Nursing. 1 year experience preferred. Required Licensure / Certification Skills: Current licensure in the State of New York.
