Minimum Requirements

Graduation from an accredited Baccalaureate school of nursing.
Completed training in the diagnosis and management of common and some complex medical conditions.
Current licensure in the State of California.
Certification as an Adult/Family/Pediatric/Women’s Health Nurse Practitioner.
Previous experience with computer-based systems is required; Electronic Medical Records experience is preferred.
A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

Euclid Street, Anaheim, CA 92801 Reporting To: Sharma Bishop Compensation: $70.00 - $82.00 / hour Description Astrana Health is seeking an NP/PA for our New Genesis Residential Care Program (RCP), a home-based primary care model serving complex, high-risk patients across San Diego. The role involves providing evidence-based, in-home care focused on improving outcomes, reducing hospitalizations, and minimizing unnecessary ER visits. The NP/PA will see an average of five patients per day, delivering comprehensive chronic and acute care in collaboration with a supervising physician, whether in the home, skilled nursing, or long-term care setting.
