Overview
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong commitment to providing quality care to patients, particularly those with developmental disabilities, dementia, and pain management needs. As an LPN, you will play a vital role in supporting the health and well-being of our patients through direct care and collaboration with other healthcare professionals.

Duties

  • Provide direct patient care, including administering medications and treatments as prescribed by physicians.
  • Assist in the development and implementation of patient care plans tailored to individual needs.
  • Monitor and document patient progress, reporting any changes in condition to the supervising nurse or physician.
  • Perform basic medical procedures such as suturing, phlebotomy, and vital signs assessment.
  • Maintain accurate medical records using electronic health record systems like Athenahealth.
  • Support patients with activities of daily living (ADLs) while promoting independence and dignity.
  • Collaborate with interdisciplinary teams in urgent care settings to ensure comprehensive patient management.
  • Educate patients and families about health conditions, treatment plans, and preventive measures.

Experience
The ideal candidate will have:

  • Previous experience working with individuals with developmental disabilities or in dementia care settings.
  • Familiarity with medical terminology and physiology knowledge relevant to nursing practice.
  • Strong interpersonal skills to effectively communicate with patients, families, and healthcare team members.
  • A valid LPN license and current CPR certification
