Overview
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong commitment to providing quality care to patients, particularly those with developmental disabilities, dementia, and pain management needs. As an LPN, you will play a vital role in supporting the health and well-being of our patients through direct care and collaboration with other healthcare professionals.

Duties

Provide direct patient care, including administering medications and treatments as prescribed by physicians.
Assist in the development and implementation of patient care plans tailored to individual needs.
Monitor and document patient progress, reporting any changes in condition to the supervising nurse or physician.
Perform basic medical procedures such as suturing, phlebotomy, and vital signs assessment.
Maintain accurate medical records using electronic health record systems like Athenahealth.
Support patients with activities of daily living (ADLs) while promoting independence and dignity.
Collaborate with interdisciplinary teams in urgent care settings to ensure comprehensive patient management.
Educate patients and families about health conditions, treatment plans, and preventive measures.

Experience
The ideal candidate will have:

Previous experience working with individuals with developmental disabilities or in dementia care settings.
Familiarity with medical terminology and physiology knowledge relevant to nursing practice.
Strong interpersonal skills to effectively communicate with patients, families, and healthcare team members.
A valid LPN license and current CPR certification
