  • JOB SUMMARY/PURPOSE
    • Collaborates with the Palliative Care Services team in the process of planning, coordinating and evaluating patient care activities to achieve department goals and maintain high quality service. Oversees and coordinates education for patients, families and staff.

    DEPARTMENT DESCRIPTION
    • Samaritan Palliative Care supports patients with serious illness and those who care for them in living with hope, resilience, and dignity throughout their journey. With attention to education, support, and resource stewardship, we address physical, social, emotional, and spiritual wellbeing. Our community-based program offers these services in the home setting which may include the patient’s own residence, skilled nursing facilities, assisted living facilities and adult foster homes.

    EXPERIENCE/EDUCATION/QUALIFICATIONS
    • Current unencumbered Oregon RN license required.
    • One (1) year experience working in a hospice or palliative care setting demonstrating working knowledge of symptom management relevant to palliative care required.
    • Case management experience required.
    • Certified Hospice and Palliative Nurse required within two (2) years of hire.
    • Healthcare Provider BLS required.
