We’re seeking a licensed Registered Nurse (RN) to join our team as a Nurse Manager / RN Clinical Liaison — a hybrid clinical and outreach role combining nursing leadership, in-home assessments, and relationship building with clients, families, and referral partners.

 

What You’ll Do

  • Conduct in-home assessments and reassessments for new and existing clients (VA and private pay).
  • Develop individualized care plans and ensure all services meet clinical, regulatory, and quality standards.
  • Supervise caregivers performing delegated tasks; ensure compliance with state and VA regulations.
  • Act as a liaison with Veterans Affairs case managers, social workers, and community partners.
  • Conduct intake and sales visits for prospective private-pay clients — assess needs, present services, and convert inquiries to admissions.
  • Train and mentor caregivers to ensure skill competency and adherence to agency standards.
  • Oversee timely authorizations and service renewals for VA and other payer programs.
  • Participate in quality improvement, incident review, and staff development initiatives.
  • Travel regularly within the service area to meet with clients, families, and referral sources. Company vehicle available for travel.

 

Who You Are

  • Licensed RN in Wisconsin and Minnesota (or compact license).
  • 2+ years of home health or community nursing experience.
  • Experience with VA programs or veteran clients strongly preferred.
  • Skilled in assessment, documentation, and care planning.
  • Outgoing and professional — comfortable meeting clients, families, and referral sources.
  • Organized, self-motivated, and able to work independently.
  • Valid driver’s license and reliable vehicle required (travel reimbursed).
  • Passionate about improving the lives of seniors and veterans.

As required by various pay transparency laws, we share a competitive range of compensation for candidates hired into each position. The pay scale is $45.17 - $65.01 USD. However, pay is influenced by factors specific to applicants, including but not limited to: skill set, level of experience, and certification(s) and/or education.
