Job Description
- Accountable for advanced nursing practice in the care of assigned patient populations. Is responsible for collecting and analyzing client health data identifying appropriate expected outcomes developing and implementing plan of care providing comprehensive clinical coordination of care and case management providing consultation to influence plan of care for clients using prescriptive authority procedures and treatment in accordance with state and federal laws and regulations evaluating clients progress in attaining expected outcomes conducting nursing research and providing education to patient family and nursing staff.
- Deliver comprehensive care to established patients including history, physical exam, treatment plan, and proper follow-up.
- Meet the patient needs by adjusting schedules and floating within the Primary Care and Endocrinology divisions.
- Participate in continuing education opportunities to maintain proficiency in evidence-based practice.
- Serve as a consultant and leader to clinical staff within the clinic.
- Participate in the call schedule (if applicable) and ensure proper functioning of telephone devices.
- Assure delivery of clinical services in a way that meets and exceeds quality standards, while providing an excellent patient experience.
- Meet or exceed the system's RVU productivity goals.
- Participate in clinic and system provider meetings to stay informed and encourage communication.
- Complete necessary patient paperwork and respond to Inbasket messages in a timely manner.
- Perform procedures that are documented on the CPA/PASA and consistent with the education, training, and experience of both the APP and their collaborating physician.
- Develop high quality visit notes with efficiency in documentation.
- Utilize critical thinking in identifying a need for collaboration with the physician.
- Maintain a Basic Life Support (BLS) certification.
- If prescriptive authority for controlled medications is determined necessary based on clinic needs, it is the responsibility of the APP to maintain all requirements for their DEA and BNDD.
- Order and interpret diagnostic studies.
- Provide personalized education to patients regarding self-management, medications, and long-term complications.
- Leverage the support of population health staff such as social work and pharmacy staff to meet patients’ medical, biopsychosocial, and financial needs.
