A Brief Overview Provides acute and rehabilitative care for patients with wound, ostomy, and continence care needs. Coordinates professional nursing and individualized, family centered care to patients with skin, wound, or ostomy issues to prevent and manage infections, reduce cost and simplify the workload of the bedside nurse using evidence based strategies for care. Essential Functions Conducts individualized wound/ostomy specific patient assessments based on consultations received from hospital staff. Assessment: ▪ Maintains expert knowledge in wound vac dressing changes and management of wound/ostomy care. ▪ Assists staff in recognizing Stage I and Suspected Deep Tissue Injuries ▪ Collects and documents data related to patient’s individualized care. ▪ Demonstrates wound/ostomy specific competency in assessment of patients by the ability to obtain and interpret information in terms of the patient needs, and understanding of the range of treatment needed by these patients. ▪ Assesses patient/family need for education and uses an individualized approach in delivery style. ▪ Assesses family expectations for, and involvement in the assessment and continuous care or treatment of the patient. ▪ Identifies nursing diagnoses, collaborative problems, and/or protocols for each patient, based on assessment. ▪ Assess and interprets patient data upon receipt of consultation. Reassessment: ▪ Performs reassessment based on team standards, CPG’s/ND’s, protocols and changes in patient condition. Communicates changes to appropriate persons (MD, Management, family) as indicated. ▪ Adjusts plan of care and treatment based on reassessment findings. Implements and evaluates nursing actions in accordance with the individualized patient/family plan of care and patient condition/needs. Oversees that the nursing and interdisciplinary plan of care/goals includes skin, wound and ostomy care. Incorporates critical thinking into the delivery of clinical skills and in analyzing patient response to treatments. Demonstrates clinical competence in interventions for skin, wound and ostomy care. Provides and/or coordinates with other disciplines patient/family education according to identified care needs, considering assessment of patient/family’s readiness to learn. Documentation reflects accurate assessment/evaluation, modification, and patient’s response to therapy, changes in condition, and therapeutic interventions/procedures. Develops wound/ostomy educational materials for health care team, patient and family: Develop and revise clinical pathways for wound care. Revise teaching sheets, policies, and CPG’s related to skin integrity, wounds, wound vacs and ostomies. Provide educational materials for patients and staff in regards to common skin, wound and ostomy problems. Provide educational offerings/presentations for staff regarding skin, wound, and ostomy care. Revise the wound and ostomy formularies to assure the organization is providing excellence in wound and ostomy care in the most cost effective manner Provides leadership in the monitoring of prevalence and incidence of pressure ulcers within the organization. Develops, enters data, and compiles reports from skin, wound, ostomy data base. Promotes pediatric research by implementing research findings into practice. Accepts assignments to share/teach knowledge and provide clinical support. Education Qualifications Bachelor's Degree From an accredited college or university in nursing Required Experience Qualifications Minimum 3 years of experience in nursing required, preferably in pediatrics Required Skills And Abilities Must be able to perform a variety of duties, often changing from one task to another of a different nature, without loss of efficiency or composure, and be able to perform under stressful conditions when confronted with multiple requests, demonstrating prioritizing skills. Staff RN - Experienced Float - Part Time/Nights

Uses available child care partners to support patient care needs as appropriate. Reviews and updates computerized care plan to accurately reflect the plan of care. Completes computerized documentation within established timeframes, including, but not limited to: Medication Administration Record, Allergies, Patient Information, etc. Completes and verifies accurate order entry in the computerized care plan. Ensures that discharge instructions clearly state patient care needs after discharge. Regular attendance at work is an essential function of the job. A Brief Overview Prescribes, delegates and coordinates professional nursing care to pediatric and adult congenital heart patients through the use of nursing process and in accordance with hospital policy/procedure. Essential Functions Implements a comprehensive plan of care for the cardiology patient/family: Teaches the patient/family about all aspects of congenital heart disease such as anatomy/physiology, medications, blood work, activities, and follow -up. Communicates the medical plan and related nursing care implementations to PCP and other medical resources involved in patient's care. Monitors outpatient lab work and reports findings to cardiologist as needed. Authorizes prescription refills under provider supervision. Provides outpatient cardiology clinic nursing coverage Reviews patient chart to assure outpatient testing is correct and appropriate. Staffs outreach clinics with cardiologists as assigned, traveling required. Acts as liaison between pediatric cardiology, patients and their families and other health care team members: Communicates the cardiologist plan of care to the patient/family. Schedules diagnostic tests and procedures when necessary, including prior authorizations. Coordinates necessary lab work, follow-up testing, and communicates any medical changes to the patient/family. Refers patient needs/concerns to the cardiologist for medical follow-up. Acts as patient advocate to address the patient’s needs and concerns. Serves as the resource for answering patient/family questions or concerns regarding medical plan of care. Works with referring physicians in coordinating and implementing patient care issues and scheduling of cardiology procedures. Attends weekly cardiac catheterization conferences in person or via zoom, attends cardiac educational offerings when appropriate Regular attendance at work is an essential function of the job. Ongoing patient/family assessments are completed according to the appropriate CPG, protocol and unit standard of care. Documentation and evaluation of nursing actions Chart audits conducted meets JCAHO and organizational expectations. Must meet overall goal of 95% (FER, Clinic problem list, signatures, pain assessment etc). Facilitates timely communication to/from patient/family and other healthcare disciplines; answers patient care related questions, requests etc. Phone calls are returned in the same business day received. Reviews patient charts as assigned to facilitate coordination of care by assuring completion of tests, consultations and procedures, interdisciplinary follow-up and referrals as needed for the clinic visit. Follows up on tests, lab results, thoroughly and within established time frames. Lab tests are followed up on within 48 hours after test results obtained. Considers cultural, cognitive, emotional, and social influences on the patient/families ability and readiness to learn. All teaching is documented on the FER to include understanding of topics taught. Provider Support Gathers patient information in a timely, concise, thorough manner and conveys this to the MD. Assists MD in clinic to maintain a good workflow and meet expectations of customers in relation to being on time. Demonstrates effective delegation techniques. Performs special tasks, procedures within the scope of practice for an RN that is unique to Ambulatory Care clinics i.e. (database entry, scheduling of special procedures, as well as assisting in special procedures, inpatient role, case management, and education/teaching). Completes with expertise, timely, organized and with detail. Communicates effectively through written documentation or verbally to appropriate individuals. Coordinates patient care to ensure continuity of care. Regular attendance at work is an essential function of the job.
