Right at Home provides in-home care for seniors and disabled adults. Our mission is to improve the quality of life for those we serve. The Lead RN Coordinator is primarily responsible for conducting initial assessments. Lead Per Diem RN Coordinators in scheduling assessments. Performs various caregiver supervisory activities by conducting supervisory visits during quarterly client reassessments. Coordinates with intake and staffing coordinators to ensure appropriate caregivers are matched with clients' needs.
ESSENTIAL FUNCTIONS
Lead team of Per Diem RN Coordinators

Supervises RN Coordinator activities
-
Ensures all relevant high-risk data is communicated to nursing staff before assessments

Schedules initial assessment and reassessment/supervisory visits
-
Conduct field orientation of newly hired RN Coordinators
-
Annual or semiannual reviews of the per diem RN Coordinators' performance
SUPERVISORY ACTIVITIES
-
Assists and conducts ongoing in-service education and compliance training of caregivers
-
Monitors caregiver job performance through skills assessment evaluations and in-home supervisory visits
CARE MANAGEMENT
-
Coordinates client services by reviewing plans of care and frequency of care
-
Engage with clients/relatives who are reported to have CICs (change in condition) by the caregiver and decide if an RN visit is necessary (before the 90-day schedule)
Other essential functions
-
Ensures compliance with company policy and all federal, state, and local regulatory bodies and conditions of participation

Remains informed and updated on home health regulations and standards

Offers suggestions in the development and implementation of policies and procedures related to RN Coordinator activities.

Perform other such tasks and assignments as directed by the management/leadership of Right at Home
EDUCATION / SKILLS / ABILITIES / AVAILABILITY
-
Registered Nurse (RN)
-
Working knowledge of HIPAA, The Joint Commission, and Medicare guidelines, and any related federal and state rules and regulations
-
Ability to effectively communicate with good oral and written communication skills
-
One year of supervisory experience required
-
Home care experience required
-
Read, write, spea,k and understand English as needed for completion of the job

Current CPR certification
-
Have a valid driver’s license and use of an insured automobile or access to adequate transportation.

Must be able to demonstrate a two-person transfer with a Hoyer Lift for educational purposes
Preferred Experience:

Excellent Documentation skills

Knowledge of common geriatric medications

G-J Tube
-
S/P Catheter care
-
G-button care and use
-
Quadriplegic experience
