Job Description: 

Summary

The Licensed Practical Nurse (LPN) performs nursing functions within the scope of practice and provides and/or coordinates care and services to meet the residents’ physical, psychosocial, and functional care needs.

Job Responsibilities

1. Practices nursing care within the scope of licensure and in accordance with the state Nurse Practice Act; Keeps a current active license; Follows state and federal licensure rules and regulations for the level of care in which they practice and in accordance with EverTrue clinical and administrative policies and procedures

2. Works collaboratively with the interdisciplinary team in defining individualized plans of care for each resident; Works closely with nursing leadership to ensure regulatory compliance; Promotes individual and team learning and provides teaching and role modeling of professional nursing practices to staff for quality clinical care

3. Passes medication and/or oversees the medication ordering and administration process; Administers medications and treatments to residents; monitors administration of I.V. fluids, supplemental nourishments, and medications when ordered

4. May initiate IV therapy if the LPN is IV-certified

5. Maintains a working knowledge and understanding of the observation/assessment and documentation process for the level of licensure in which they work, including care and service plans and the MDS as applicable

6. Utilizes the electronic medical record to capture full clinical documentation in accordance with policy and procedures 

7. Observes resident status as necessary to assure quality care; Coordinates with other resident services to assure continuity of care; Documents medical information and maintains detailed, timely, and accurate reports

8. Maintains a safe, comfortable, and functional environment by assessing potential hazards; Implements practices to maintain optimum safety

9. Oversees and directs the certified staff and others as assigned to ensure the EverTrue standards of care and services are provided

10. Reports and supports investigative protocols as it relates to staff or resident incidents and/or accidents in accordance with state and federal regulations

Qualifications, Knowledge, Skills & Abilities

  • LPN licensure in state of practice

  • Must be CPR certified

  • Specialized training or experience in gerontological nursing preferred

EverTrue Pay & Benefits

Pay grade: 12, $23.55/hour to $37.75/hour. Eligible for shift differentials: Yes. $2.00/hour evenings; $1.50/hour nights; $1.50/hour weekends (States of IL & MO; May vary in other states)
