Overview

Community Health Partners (CHP) is seeking a full-time Nurse Practitioner (NP) or Physician Assistant (PA) to join our well-established outpatient endocrinology practice in Fresno, CA. This is a unique opportunity to work alongside a collaborative team of four Endocrinologists and two experienced Advanced Practice Providers (APPs) in a thriving and comprehensive center dedicated to providing exceptional endocrine care.

Responsibilities

Accountabilities1. Provides direct assessment, evaluation and primary patient care, complying with medical protocols and procedures.2. Provides direct patient management in collaboration with physicians; participates in daily rounds, patient assessments and care plan development.3. Orders, interprets and evaluates diagnostic tests to identify and assess patient's healthcare needs. Furnishes medications based upon written protocols.4. Ensures that patients are counseled regarding treatment plans and the use, dosage and possible side effects of medications.5. Makes referrals to appropriate providers when patients require subspecialty medical attention beyond the area's scope of service.6. Obtains prior authorizations from insurance companies.7. Documents and maintains precise and complete records of patients.8. Distinguishes between normal and abnormal findings.9. Provides education, training, information and consultation services to interdisciplinary team, patients, family and caregivers.10. Participates in quality assurance activities, including assistance in development and implementation of new policies and procedures, recommends updates to current policy as needed.11. Performs other duties as assigned.

Qualifications

Education• Completion of a Nurse Practitioner or Physician Assistant program required• Credentialed by the CMC Medical Staff Office as an Allied Health Professional and privileged to perform Nurse Practitioner or Physician Assistant Standardized Procedures requiredExperience
