Position Summary

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our hospice team serving the Montgomery County, Pa area. In this role, the LPN provides high-quality, comfort-focused nursing care to hospice patients in their homes and other care settings, under the direction of a Registered Nurse and in collaboration with the interdisciplinary care team.

Key Responsibilities

  • Provide skilled nursing care to hospice patients in accordance with the individualized plan of care
  • Perform nursing tasks within LPN scope of practice under RN supervision
  • Monitor and document patient status, symptoms, and responses to care
  • Assist with medication administration and medication reconciliations as ordered
  • Provide wound care and other treatments per physician and RN direction
  • Educate patients and families on daily care needs and comfort measures
  • Communicate changes in patient condition promptly to the RN case manager
  • Maintain accurate, timely documentation in compliance with hospice regulations
  • Support patients’ comfort, dignity, and emotional well-being

Qualifications

  • Current Pennsylvania LPN license in good standing (required)
  • Hospice, home health, or long-term care experience preferred
  • Strong clinical assessment and communication skills
  • Compassionate, patient-focused approach to care
  • Ability to work independently in the field
