Position Summary: Provides skilled nursing care to patients in Skilled Nursing Facilities, Hospitals, and Independent Living Communities along various Healthcare Continuums as needed. Coordinate care with the patient, family, referring agency, and assumes responsibility for the coordination of care. Provides service in accordance with the Agency philosophy, standards and policies.
