QUALIFICATIONS:

  1. Must have at least one year of experience with in home care, private-duty home care setting, community, or have at lease one year of verifiable work experience in an unrelated field and if in an employed state where necessary, have successfully completed a Home Health Aide Training course.
  2. Demonstrate an ability to effectively communicate in English, and/or the predominate language of the population being serviced and must be able to read and write.
  3. Pass the Spring Hills Home Care Services Competency Evaluation
  4. Demonstrates competency in performing all tasks
  5. Pass a criminal background

  7. Have reliable transportation and method of communication
  8. Is punctual, dependable and demonstrates sound judgement

JOB RESPONSIBILITIES:

  1. Follows the established Plan of Care.
  2. Assists with activities of daily living means the functions or tasks for self-care which are performed either independently or with supervision. Activities of daily living include mobility, transferring, walking, grooming, bathing, dressing, undressing, eating, and toileting, per the Plan of Care.
  3. Assists with “Independent Activities of daily living” which may include light housekeeping, shopping, laundry and meal preparation, per the Plan of Care.
  4. Accompany clients to appointments (transportation is client’s responsibility unless driving waiver is signed and per the Plan of Care)
  5. Provides assistance with self-administered, pre drawn and medication reminders, per Plan of Care
  6. Understands and maintains infection control and safety measures
  7. Reports any and all changes in the client’s condition to the Care Manager or BOM
  8. Completes the Activity Records and accurately documents the care provided
  9. Performs all activities as directed by the Care Manager or BOM
  10. Has a positive relationship with clients and families
  11. Has positive client satisfactions
  12. Meets all deadlines for health screening requirements 
  13. Encourage maximum independence to facilitate rehabilitation as determined 
  14. Respect client’s rights and ensure confidentiality
  • The PCA/HHA/Home Care Aide job performance is evaluated 90 days into employment and yearly by Care Manager or BOM using input from clients, coordinators and personal
  • This job description is used as a guide each PCA/HHA/Home Care Aide is responsible for complying with all SHHCS’s policies

JOB DESCRIPTION Job Summary Provides support for care management/care coordination activities and collaborates with multidisciplinary team coordinating integrated delivery of member care across the continuum. Essential Job Duties • Completes assessments of members per regulated timelines and determines who may qualify for care coordination/care management based on triggers identified in assessments. • Develops and implements care plan in collaboration with member, caregiver, physician and/or other appropriate health care professionals and member support network to address member needs and goals. • Conducts telephonic, face-to-face or home visits as required. • Performs ongoing monitoring of care plan to evaluate effectiveness, document interventions and goal achievement, and suggest changes accordingly. • Maintains ongoing member caseload for regular outreach and management. • Promotes integration of services for members including behavioral health, long-term services and supports (LTSS), and home and community resources to enhance continuity of care. • Facilitates interdisciplinary care team (ICT) meetings and informal ICT collaboration. • Uses motivational interviewing and Molina clinical guideposts to educate, support and motivate change during member contacts. • Assesses for barriers to care, provides care coordination and assistance to member to address concerns. • Collaborates with licensed care managers/leadership as needed or required. • 25- 40% estimated local travel may be required (based upon state/contractual requirements).
