Key Responsibilities: The Admissions/Discharge RN plays a critical role in ensuring seamless, safe, and thorough transitions for our residents. Discharge Planning: Responsible for assuring a safe and thorough discharge for residents transitioning to home or other levels of care. Coordination: Securing Durable Medical Equipment (DME), contacting external agencies for transitions of care, and coordinating with medical providers. Documentation & Compliance: Reviewing discharge forms, working to streamline forms and processes for future use. Quality Improvement: Completing discharge phone calls for readmission tracking and Quality Assurance and Performance Improvement (QAPI) purposes. Collaboration: Working collaboratively with the interdisciplinary team; will serve as a social worker representative for Rehab residents, complete PASARRs, and attend care conferences and Medicare meetings. Training: Training other nurses as directed by the Director of Nursing (DON). Qualifications: License: Current Registered Nurse (RN) license. Experience: Minimum 3 years of nursing experience, preferably in long-term care or hospital settings. Knowledge: Strong knowledge of care planning, complex discharge protocols, and regulatory requirements. Skills: Excellent communication, organizational, and interpersonal skills. Ability to work collaboratively with interdisciplinary teams. Preferred: Experience with the Rehabilitation population is a plus. Familiarity with transitions of care.
