Cooperative decision making is expected in the execution of duties. Essential Functions Implements and evaluates patient care under the supervision of an RN. Participates in the development of a patient-specific care plan. Provides direct patient care in patient homes including administering medications and treatments as prescribed. Understands and applies knowledge in edema/lymphedema management, venipuncture, management of PICC lines, implanted ports, post-surgical drains, and pulmonary and abdominal drains. Provides focused systems and wound assessment. Reports acute medical problems and condition changes to the case manager and/or attending physician, as directed. Demonstrates proficiency in communicating effectively with individuals diagnosed with mental and behavioral disorders, and to operate efficiently in an independent home setting. Documents patient care in health record according to department standards. Delegates, prioritizes, requests and accepts guidance and assistance from others to assure delivery of care in a safe and timely manner Ensures own professional effectiveness through continued education and professional development. Assists in the transporting of patients, specimens, supplies and equipment and in the maintenance and stocking of supplies and equipment to patient homes. May triage patient phone calls, counsel patients, providing patient education, taking patient histories, administering prescribed medications, changing dressings, cleaning wounds and monitoring patient vital signs. Teaches the patient and caregivers about disease management, in accordance with the plan of care. Performs other duties as assigned. Qualifications Education Accredited State Board of Nursing LPN Program Required: Experience Minimum of 1 year Preferred: Clinical experience as an LPN and Preferred: Demonstrated knowledge of and ability to apply age specific principles of growth and development and life stages to meet each patient’s needs Credentials Required: Upon Hire Licensed Practical Nurse - Oregon or Required: Upon Hire Basic Life Support and Required: Upon Hire Driver's License and proof of insurance Skills Basic computer skills required including familiarity with MS Office applications. The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc. Job Summary Responsible for providing direct skilled nursing care and coordination/management of patients admitted to home care/clinic services using considerable judgment and skill in making patient care decisions. Utilizes the professional healthcare process of assessment, problem identification/planning/intervention and clinical/goal outcome evaluation. Essential Functions Perform initial evaluation of patient/home and apply admission criteria to determine if patient is safe candidate for home health/hospice services. Provide skilled nursing care to patients and skilled education/instruction to patients and caregivers according to the home health/hospice plan of treatment. Performs physical assessments, develops, coordinate, modifies and manages written individualized plan of care, as ordered by the primary physician, to meet patient’s long and short-term medical goals and appropriate/timely home health outcomes. Develops and clinically manages the Home Health Aide’s care plan according to the patient’s evolving personal are needs. Coordinates care, collaborates and communicates with other members of the interdisciplinary team to assure appropriate and smooth flow of care/services through the care continuum. Performs discharge planning appropriately towards patient specific goals/outcomes and evaluate interventions and outcomes of patient care by reviewing long and short-term goals established in the plan of care.
