Administers medications, treatment, and provider-specific care to patients in compliance with nursing policies/procedures, LPN scope of practice, and as applicable to assigned area(s). Communicates all appropriate and critical changes in the patient’s condition, status, and findings to the appropriate stakeholders in a timely manner and as defined by policies and procedures and/or provider orders as applicable to assigned area(s). Documents all clinical findings and required regulatory mandates in the medical chart in compliance with policies and procedures or as defined by regulatory requirements as applicable to assigned area(s). Contributes to maintaining an updated, individualized plan of care that includes; goals, interventions, medications and visit frequency orders as applicable to assigned area(s). Maintains a thorough knowledge of infection control procedures and strictly adheres to policies and procedures.
