Under supervision of the Registered Nurse and/or Nurse Practitioner, this position involves reviewing patients' lab results, contacting patients to communicate findings, offering educational support on specific disease processes, and coordinating necessary follow-up appointments. The nurse will work collaboratively with the Wellness for Life team, demonstrating leadership and participating in community nursing education and wellness initiatives. Qualifications: 1. Active and unrestricted Licensed Practical Nurse (LPN) license in Louisiana. 2. Minimum of 2 years of nursing experience, preferably in wellness or community health is preferred. 3. Strong knowledge of lab result interpretation and disease processes preferred. 4. Excellent communication and interpersonal skills. 5. Ability to adapt to changing demands and work collaboratively within a team. 6. Demonstrated leadership skills in patient education and community outreach.
