Job Details Description Position Summary: A team-based care RN (Registered Nurse) navigator plays a pivotal role in delivering coordinated, patient-centered healthcare within a multidisciplinary team. Their responsibilities emphasize collaboration, evidence-based practice, and quality improvement. The RN Navigator is an experienced registered nurse who is accountable for the effective clinical care coordination of patients. The Navigator proactively identifies and resolves patient’s needs. and clinical guidance throughout their care within Deborah Heart and Lung Center and it's practice locations.
