Position Summary: Provides direct patient care and comprehensive coordination of care for patients and their families/caregivers. Eliminates barriers to timely care, facilitates flow through system, facilitates interactions with clinical research office, increases patient and provider satisfaction, maintains point of contact with providers and serves as the first point of contact for patients and families. Performs nursing assessments and triage of patient care needs for new and ongoing patients, including patients on a clinical research study. Oversees the planning, scheduling, and implementation of day-to-day clinical activities and procedures for navigated patients. Works with physician and clinical research office to facilitate, coordinate and ensure patient access to clinical trials where appropriate. Central point of contact for all navigated patients; including interaction with medical, nursing, ancillary services, and when appropriate the clinical research services. Provides education to patients, families, and significant others; acts as an information resource to students, health care professionals, patients, and the public. Collects and reports data from patient on barriers, health care, medications, prior treatments, etc. Tracks order completion and follow up. Monitors patient compliance with patient treatment plan. Assists patients with referrals to outside resources and care coordination as needed. Designs or participates in the creation of health education materials. Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies. May develop and conduct in-service training programs. Complies with federal, state, and local legal and certification requirements by studying existing and new legislation; anticipating future legislation; enforcing adherence to requirements; advising management on needed actions. During intraoperative care, serves as the first assistant to the surgeon during the surgical procedures. Makes daily rounds with or without physicians, writes progress notes and discharge summaries to facilitate patient care and ensure timely discharge of the patient. Responsible for quality assessment activities, including monitoring and sharing of key quality patient care metrics through continuous quality improvement. Performs accurate and thorough documentation meeting all core measures and STS (Society of Thoracic Surgeons) standard of care elements. About the unit Neuroscience nurses provide care for patient with a various illness of the brain and nervous system in this 32-bed neuro-medical unit. Patient diagnosis include but are not limited to CVA, TIA’s, Parkinson’s disease, Seizures, Multiple sclerosis and other Neurological disorders. Nurses will also be caring for other general medical surgical patients with various illnesses in this area. The unit has 12 telemetry beds for patients requiring close cardiac and neurological monitoring. Scope of work The RN, under the ANA scope of practice utilizes the framework of the nursing process to provide professional nursing care by incorporating patient-family centered care principles to assess, plan, implement, evaluate, and discharge. The RN provides quality patient care under the direction of the Nurse Leader according to Corewell Health policies and procedures and recognizes the impact of nursing care and patient/family partnerships on patient outcomes. The RN partners with and utilizes patient/family input and goals in planning plan of care and develops teaching strategies for patient/family; documents education and learning appropriately in health record. Qualifications Associates in Nursing Required Bachelors in Nursing Required within 5 years Graduates from a non-accredited school of nursing must have at least one year of acute care experience or at least two years of experience in a licensed health care role and must have successfully passed the NCLEX licensure examination.
