Assessment and Care Planning: Conduct initial and ongoing assessments of patients' health status. Develop and implement individualized care plans based on these assessments to meet the patient's needs. Medication Administration: Administer medications and treatments as prescribed by the physician. Monitor and assess the patient for any adverse reactions or side effects. Wound Care: Provide wound care management, including dressing changes, infection control, and monitoring healing progress. Patient Education: Educate patients and their families about their medical conditions, medications, and treatment plans. Teach them techniques for self-care and management of their health at home. Monitoring and Evaluation: Monitor patients' progress and condition regularly. Evaluate the effectiveness of the care plan and make necessary adjustments based on patient outcomes. QUALIFICATIONS Graduate of an approved school of nursing or Bachelor’s degree of Nursing from an accredited college/university Currently licensed, in good standing, to practice in New York as a Registered Nurse. Knowledge of nursing and medical practices, procedures, regulations and guidelines pertaining to home care, including OASIS. Complies with accepted professional standards and practice.
