The Transitional Care Team (TCT)- Registered Nurse (RN) will ensure a smooth transitions for our PICU and NICU patients from the hospital to the home. They will provide education and training for the parents and family as well as train and facilitate any orientation for new caregivers. Salary: $30 - $32 / per hour Responsibilities Utilizes the nursing process to assess, plan, implement and evaluate patient care. Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition. Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses. Documents the patient’s plan of care using identified nursing diagnoses, expected patient outcomes, and selected nursing interventions. Performs interventions according to identified priorities, plan of care, and the hospital policies and patient care outcome standard. Revises the plan of care according to evaluation, changes in medical plan of care, and effective/ineffective nursing interventions. Uses clinical judgment in evaluation activities to meet patient care needs of an assigned unit/floor including establishing priorities. Other Registered Nurse (RN) duties as assigned. This position will work in a multidisciplinary team to innovate, improve, and advance the quality of care through a patient centered approach. Essential Functions Delivers care in a nondiscriminatory manner that is sensitive to and values diversity and serves as a liaison between patients, their families, and members of the healthcare team. Provides chronic care management services on a regular basis to assess patient compliance to care plan, health status by completing a nursing assessment; Performs telephone triage, explains, and confirms understanding of prescribed treatment regimens/plan of care and confirms compliance to same, notifies appropriate physician and/or advanced practice provider of abnormal findings and coordinates with other disciplines to ensure patients receive appropriate and timely care. Assists Registered Dieticians in addressing patient and family needs while in the hospital and post discharge. Identifies any nutritionary issues, such as non-compliance with recommended nutrition guidelines and communicates issues to the Registered Dietician and other members of the care team. Assists Clinical Psychologist in addressing patient and family needs while in the hospital and post discharge. Participates in quality initiatives, process improvements, and research activities to improve patient outcomes, patient experience, and patient safety. Including, but not limited to data collection in support of clinical cardiology and cardiothoracic research including data collection and input of data into the NPC-QIC Collaborative database. Meets with all patients prior to planned surgical admissions to establish repour and assists patient by facilitating presurgical admission activities to ensure a smooth flow and exceptional patient experience during the process.
