The Telemetry Unit provides a comprehensive team approach to medical nursing and psychosocial needs of patients and families. The nursing ratio of 1:4-5 allows the patient’s physical, psychological, and social needs to be addressed. The Telemetry Unit is comprised of Medical, Cardiology, and Intermediate Care patients. Common diagnosis includes CHF, Cardiac Dysrhythmias, Acute Coronary Syndrome, Myocardial Infarction, Pacemakers, and Cardiac effusions. Nurses will care for patients Pre/Post Cardioversions, Cardiac Catheterization, and Stress Tests. Penn Medicine Princeton Health (PMPH) is seeking a full time Registered Nurse (RN) to work the day shift within the Radiology Nursing department. Interventional Radiology is a medical subspecialty that performs minimally invasive procedures using imaging guidance such as MRI, CT, ultrasound and fluoroscopy. The Interventional Radiology department provides a safe and comfortable environment for both patients and personnel. Patients can expect care to be professional and timely. The patient’s physical, psychological, and social needs will be addressed. Patients are initially assessed on the inpatient nursing unit or perioperative unit and are reassessed on arrival to the IR suite. Patients are continually reassessed throughout the pre-, intra-, and post-procedure periods. Modifications to the plan of care are based on reassessment of the patient. Scheduling Requirement: 4, 10 hour day shifts a week. Weekend requirement: 1 weekend per month. On call requirement Key Roles and Responsibilities: The Radiology RN provides moderate sedation and analgesia during the procedure and monitors the patient throughout the procedure. The Radiology RN is involved in the planning of the procedure, pre-procedure preparation, intraprocedural monitoring of patient and post-procedure. Radiology RNs provide education to the patient as they go through all the phases of care. The SCU (Surgical Care Unit) is a 36 bed medical-surgical unit with a specialized focus on patients receiving inpatient surgical interventions. A portion of the unit, the Jim Craigie Center for Joint Replacement, is a dedicated use area for total hip and knee replacement patients. The Surgical Care Unit is certified as a Bariatric Center of Excellence and Joint Commission Advanced Hip and Knee Replacement Center. Scheduling Requirements: You will be required to work three (7PM-7AM) night shifts per week. You will also be required to work at least three weekend shifts per 4-week schedule. Key Roles and Responsibilities: Nurses on SCU will provide pre and post-operative care for patients recovering from a variety of surgical procedures (hip fractures, spinal surgery, other orthopedic surgeries, bariatrics, various abdominal surgeries, breast surgeries, urological interventions, etc.) Emphasis on patient and family education to ensure clear expectations and optimal outcomes during surgical recovery, both in the hospital and at home Some of the clinical skills performed on a daily basis consist of: safe patient ambulation, pain management, drain care, nasogastric tube care, foley catheter care, chest tubes, central lines, wound vac and wound care, complex medication administration, blood administration and constant communication with the interdisciplinary team. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 7. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. 8. Counsels the patient and family in meeting nursing and related needs. 9. CentraState Medical Center is currently seeking a Registered Nurse in the Emergency Department. This role administers culturally sensitive nursing care to assigned patients that is in accordance with ANA standards, the NJ Practice Act, Joint Commission, professional standards and established hospital and Nursing Department policy and procedures in accordance with evidence based nursing practice. Utilizes the Nursing Process in providing patient centered care. Evaluates the effectiveness of nursing care through the analysis of nurse sensitive indicators and patient outcomes. Orientation will include satisfactory completion of CPR Certification, IV Course, and Departmental Orientation. A patient focused care model utilizing a multidisciplinary approach is used as the method to deliver patient care. Orientation and annual renewals will include satisfactory completion of non-violent crisis intervention. Responsibilities Provides considerate respectful care focused upon the patient's individual needs to ensure patient satisfaction. Affirms the patient's right to make decisions regarding his/her medical care, including the decision to discontinue treatment, to the extent permitted by law. Successfully completes life-span education as appropriate. Obtains a complete nursing history and baseline assessment that is systematic, consistent and continuous. This assessment is communicated effectively utilizing Hand-off policy and documented. Documents results of patient assessment, nursing diagnosis, care plan nursing interventions and evaluation and patient progress in medical record according to hospital/nursing policy and the Nursing Standards of Care/Practice for the specific patient type. Appropriately analyzes patient information /assessment to determine nursing diagnosis and needs for appropriate nursing care, medical intervention or services of other clinical professionals. Identifies patient problems and formulates nursing diagnosis according to physical, psychological, spiritual/cultural and educational needs in conjunction with a multi disciplinary approach. Consistently includes patient and family in the development of an individualized plan of care. Administers medication as needed following CSHS policies and manufacturer's instructions. Takes immediate and appropriate actions in the event of a medication variance occurrence. Recognizes changes in patient condition and/or emergency situations and takes immediate and appropriate actions. Participates in unit conferences to discuss quality issues and quality improvement strategies. Offers ideas to improve processes for patient safety. Delegates components of care to non-professional caregivers as appropriate. Follows up on any delegated components of care and assignments given. Makes decisions that demonstrate knowledge of nursing practice; utilizes sound nursing judgment in clinical and patient care management situations. Demonstrates critical thinking skills in the care of patients and in the coordination of care to assure the best patient outcomes. Follows CSHS Pain Assessment policy. Successfully functions as a Resource Nurse, making decisions in the absence of the Clinical Leader and/or the Nurse Manager. Supports cost containment through efficient use of equipment, supplies and other pertinent resources. Recognizes adverse reaction to medications and takes immediate and appropriate action. Evaluates the patient's needs for discharge and accesses appropriate resources for the patient and/or family. Consistently provides written and verbal education to patients and/or families. Successfully functions as a preceptor/mentor to students and new staff members. Consistently promotes teamwork within the unit and between departments. Consistently adheres to hospital policy regarding the insertion, maintenance and removal of foley catheters, if applicable. Able to verbalize and demonstrate proper rationale and techniques surrounding all aspects of indwelling urinary catheter care as per policy #4668. Fulfills the requirements for competencies and completion of annual mandatories surrounding indwelling urinary catheters. Qualifications Bachelor’s Degree in Nursing preferred. Previous related experience preferred. Current NJ RN license required. CPR Certification required. Stroke education requirements to be achieved within first year then maintained thereafter. Specialty credentialing required to be achieved within the first year and maintained thereafter include Pediatric Advanced Life Support (PALS), Advanced Cardiac Life Support (ACLS), 8 hours Trauma Education, CPR and CSMC IV Certification. Specialty professional nursing certification preferred. Must complete assigned education provided by Crisis Prevention Institute. Strong professional, organizational, and interpersonal skills to effectively relate with all members of the healthcare team.
