Job Details Care coordination for a multisite Endocrine practice managing the full spectrum of adult Endocrine diseases including the management of complex, high risk diabetic patients. The RN will develop a close working relationship with the Physicians and Nurse Practitioners to facilitate patient care, to address and support patient needs and to enhance the quality and timeliness of care as well as the patient experience. The RN will provide services for Endocrine patients including but not limited to: performing direct patient outreach to close gaps in care, hospital follow-up, educating patients on various components of their care plan, bridging communication between outpatient visits, responding to patient questions and access issues to ensure that patients receive timely care and instruction. Work closely with patients, their caregivers and 3rd party payers to assure the execution of their treatment plan. Act as a clinical resource to providers and staff. Work in collaboration providers, support staff and others to facilitate clinical tasks, workflows and patient education. Demonstrate excellent practice skills including leadership, flexibility, priority setting, problem solving, conflict resolution, decision-making, organization and verbal/written communication skills. Job Description Education/Training/Experience: -Current license to practice as a Registered Nurse in the State of Pennsylvania is required. BSN or actively pursuing a BSN degree preferred. -Willingness to pursue and complete Certified Diabetes Educator (CDE) credential required. -Minimum 3 years of experience as an RN, preferably with adult patients with chronic diseases such as diabetes, hypertension, hyperlipidemia, obesity, etc. -CPR certification is required. -Computer skills are required. -Knowledge/familiarity with ICD10 codes, CPT coding, and medical terminology is required. -Ability to perform advanced clinical functions, including but not limited to: venipuncture, injections, EKGs and medication administration are required. -General typing (28 WPM) preferred; filing and reception skills, spelling, grammar and telephone etiquette skills, and strong customer service skills are required. Responsibilities: The Hospital at Home (H@H) Resource Nurse competencies and accountabilities are consistent with the Clinical Level II Nurse. The H@H Resource Nurse is an RN that practices as a full partner on the health care team and consistently demonstrates safe practice within the organizational professional practice model. H@H In-Home Nursing: Conducts home visits per schedule defined by nursing care plan (at lease 2/24 hours per patient) and in response to urgent need (within 30 minutes). Conducts initial and ongoing home and patient safety assessments, provides intervention should the home environment change or pose a safety risk for patients or staff. Resource Nursing: Conducts home visits for Penn Home Infusion Team and Penn Medicine Home Health when operationally appropriate to fill in for vacancies or other operational needs. Responds to alerts, communicates directly with patients to resolve alerts.
