Previous hospital and/or health plan experience with Discharge Planning desired, and knowledge of post-acute admission criteria (SNF, AR, LTAC). This role would be a hybrid position once acclimated, with one day a week onsite with occasional site visits within the area. Summary Perform duties to conduct and manage the day-to-day operations of the utilization management function for post-acute utilization services. Communicate with staff to facilitate daily department functions including prior authorization for post-acute services related to Long Term Acute Care Hospital (LTACH), Acute Rehabilitation (AR), and Skilled Nursing Facilities (SNF), Discharge Planning and transitions of care. Promotes coordination of members to clinically appropriate settings, and appropriate referrals to SummaCare programs. Meets organizational, operational and service goals related to quality outcomes, member experience, and cost-efficiency. Formal Education Required Bachelor’s degree or equivalent combination of education and/or experience Licensed as a Registered Nurse in the state of Ohio Case Management Certification (CCM, ACM) preferred Experience And Training Required Three (3) years performing post-acute utilization management/ discharge planning activities and minimum of four years relevant clinical experience. Experience including prior authorizations, post -acute clinical nursing, health plan management, working knowledge of Medicare and/or Commercial plan designs, and/or experience with clinical regulatory and accrediting guidelines preferred Prior management/leadership experience required, preferably in post-acute areas and/utilization management, or health plan management. The 8100 Family Practice Unit delivers a range of neuro, cardiac telemetry and general medicine services for populations presenting with various diagnoses, such as sepsis, pneumonia, strokes and FFT. As an RN on this team, you will assist fellow caregivers in delivering exceptional care, from initial introduction to final send-off. In this role, you will interact directly with patients, families and visitors, offering the opportunity to enhance their experience and supply healthcare education as needed. Additionally, the Cleveland Clinic system provides nurses with many professional development resources, including furthering education and online/in-person courses to enhance your skills. A caregiver in this position works nights from 7:00PM - 7:30AM including every other weekend and 3 holidays per year. A caregiver who excels in this role will: Complete ongoing assessments. Recognize significant changes in patient condition and respond appropriately. Develop and update goals for identified problems. Involve the patient/family (or designee) in developing a plan of care individualized to patient cultural, religious and educational needs. Coordinate the interdisciplinary plan of care. Serve as a patient advocate. Safely administers patient care, medications and treatments. Document legibly using concise terms, correct terminology and approved abbreviations. Participate in on-going interdisciplinary evaluation of patient/family outcomes and contribute to the improvement of care. Delegate tasks/assignments to employees according to their ability and within the scope of their job. Minimum qualifications for the ideal future caregiver include: Graduate from an accredited School of Nursing Current state licensure as a Registered Nurse (RN) Basic Life Support (BLS) Certification through the American Heart Association (AHA) Preferred qualifications for the ideal future caregiver include: Bachelor’s of Science in Nursing (BSN) Our caregivers continue to create the best outcomes for our patients across each of our facilities. Description A Brief Overview Provides safe efficient age specific nursing care to patients/families using the nursing process in order to assist them to attain, maintain, regain, and sustain optimal health status. What You Will Do Implements the nursing process by delivering effective relationship based nursing care. Demonstrates technical skills in care delivery that result in safe practice and positive care outcomes.
