Overview The Department of Medicine has developed a Comprehensive Sickle Cell Program in collaboration with the Departments of Physical Medicine and Rehabilitation and Psychiatry. This program is supported by EVMS and Sentara Health. The goal of this program is to provide top-notch outpatient and inpatient clinical care to patients with sickle cell disease. We are looking for Advanced Practice Providers (APP), a nurse practitioner (NP) or a physician assistant (PA), who will function as a clinical provider in the Division of General Internal Medicine, Department of Medicine. The APP will participate in all aspects of patient care and will participate in the supporting roles of consultant and educator as indicated. The clinical practice will mostly focus on clinical care of patients with Sickle Cell Disease in the hospital and in the outpatient clinic under supervision of clinical faculty. The clinical practice may also include provision of primary care to all types of patients. Responsibilities Independently assess, diagnose, & develop treatment plans for patients with sickle cell disease as well as patients in the area of Primary Care in accordance with protocols and standards of care. Promote a schedule of health promotion examinations and interventions in our patient population Obtain medical histories, perform physical examinations, and prescribe, dispense or deliver medications under a collaborative practice agreement. Differentiate between normal and abnormal findings and orders appropriate diagnostic studies. Provide anticipatory guidance and counseling to patients/families. Document patient assessment and plan of care in the medical record and generate appropriate outgoing correspondence. Be familiar with documentation requirements to insure proper billing. Initiate and coordinate referrals to other health care team members or agencies. Serve as a consultative resource to nursing staff. Participate in the development, utilization and evaluation of resources to meet patient/family educational needs. Provide education to external agencies, community and other health professionals. Participate in development and implementation of divisional and departmental quality assessment/improvement/research programs and processes. Monitor task module of the electronic health record to insure timely response. Responsible for administrative activities and special projects as assigned. Contribute toward the achievement of divisional and departmental goals. Participate and contribute to activities that enhance clinical expertise and nursing knowledge. In this dynamic position, APP will take charge of managing health issues and coordinating healthcare in progressive care or acute care in-patient hospital settings. As an integral part of our team, the APP holds the responsibility of delivering top-notch patient care and medical services to individuals, families, and/or groups within the hospital setting. This includes working independently and collaboratively with a sponsoring physician, and internal medicine residency program teaching teams, providing a wide range of health care services. The scope of practice encompasses conducting thorough health assessments, diagnosing medical conditions, developing personalized medical plans of care, and prescribing/providing treatments and interventions. APP will also play a key role in evaluating patient responses, making referrals, and adeptly managing both acute and stable chronic illnesses. Overview Under the daily supervision of the Practice Manager, and Clinical Supervision of a Psychiatrist, this role functions as a physician assistant within EVMS Department of Psychiatry and Behavioral Sciences. This role will enable the Department Psychiatrists to expand their capacity to perform new patient comprehensive evaluations, and to provide continuous patient care for patients that have established and determined diagnosis. Utilizes DSM5 principles to modify patient diagnoses and treatment under the direction of a supervising psychiatrist. Overview Advanced Practice Providers (APP), a nurse practitioner (NP) or a physician assistant (PA), who will be responsible for providing general medical care and treatment of participants enrolled in clinical trials in the Department of Obstetrics and Gynecology at Macon & Joan Brock Virginia Health Sciences. Position functions as an independent provider primarily coordinating gynecologic and obstetric care in women enrolled in clinical trials. This professional will work with the research staff on protocol review, study activities, and review and follow-up of diagnostic and laboratory tests. Responsibilities Performs physical examinations, protocol procedures (for example endometrial biopsies, cervico vaginal biopsies) and preventive health measures within prescribed guidelines and instructions of a physician. These measures include but are not limited to health maintenance, pelvic exams, PAP smears, education and counseling. Perform an individualized assessment of the research participants. Document physical findings in study source documents and or case report forms. Work with research coordinators and staff to troubleshoot daily study issues, and respond to sponsor queries and deadlines. Some research participants may need to be seen on weekends and or holidays. Prescribes or recommends drugs in accordance with state laws, or other forms of treatment such as physical therapy, inhalation therapy or related therapeutic procedures. Orders, interprets and evaluates diagnostic tests to identify and assess patient's clinical problems and health care needs. Provides diagnosis and treatment of common acute illnesses within prescribed guidelines and instructions of a physician, as well as education, counseling and crisis intervention as they relate to acute care. Records all pertinent patient information in the Electronic Medical Record (EMR), study source documents or case report forms.
