Overview Responsible for providing prenatal, postnatal, general medical care and treatment of patients within the Maternal Fetal Medicine division which is a division of the Department of Obstetrics and Gynecology at Eastern Virginia Medical School. Nurse Practitioners are expected to function as an independent provider primarily coordinating pre-pregnancy, prenatal and post-partum care in women with high-risk pregnancies. This position will provide a combination of inpatient and outpatient care duties. Maternal Fetal Medicine nurse practitioners’ function in an expanded nursing role and perform selected tasks and services within the province of medicine as well as tasks that fall within the province of nursing. Nursing and medicine must maintain ongoing collaboration in education and practice so that the nurse practitioner is maximally effective in meeting the primary health-care needs of women. When performing nursing aspects of this role, the practitioner must have the opportunity to engage in independent as well as co-operative decision-making. When performing expanded aspects of the role that overlap with medicine, the practitioner must function in collaboration with qualified physicians within the framework of jointly approved guidelines. Nurse practitioners not only must be prepared and competent to apply additional knowledge and skills, but also must be ready and willing to accept responsibility and accountability for their practice. This position will require travel between all Maternal Fetal Medicine locations and Sentara Norfolk General Hospital. Responsibilities Obtain a complete health, psychosocial, obstetric, and gynecologic history and record findings in a systematic, accurate, and succinct form Perform a complete physical examination, with specific emphasis on the reproductive system Order, perform, and interpret routine laboratory tests Develop and implement plans for health promotion and health maintenance, including plans for disease prevention and provision of health education and counseling Provide periodic health assessment of normal, non-pregnant, and asymptomatic women Provide clinical management of women having gynecologic problems Provide assessment, education, and management for family planning and fertility control Provide prenatal care for women with pregnancies, giving attention to both maternal and fetal health Provide postpartum care, including attention to parent and infant relationships, for women who have had pregnancies Provide health education and counseling, with emphasis on physical and psychosocial dimensions of adolescence, sexuality, childbearing, parenting, climacteric, aging, and family life Co-manage care for patients with selected health complications in collaboration with the physician Arrange referrals to other members of the health-care team as needed and to community resources as appropriate Qualifications Required: Nurse Practitioners must have a MSN with a nurse practitioner focus. Primary care is provided at all levels of the health-care system, but predominately in an ambulatory setting. The obstetric-gynecologic/women’s health nurse practitioner provides such care in collaboration with physicians, as well as with other members of the health-care team. Position supports all office locations. Practitioner will be required to travel between all office locations. Position will work directly with the Minimally Invasive Gynecology Surgery division within the department of Obstetrics and Gynecology. Responsibilities Provide diagnostic and therapeutic healthcare for complex gynecological conditions, such as endometriosis and abnormal uterine bleeding. Perform history and physical examinations, initial medical decision making and perform minor office procedures. Educate patients as to the nature of disease and provide instruction on proper treatment and care. Provide general OBGYN principles and practices to patient care. Obtain a complete health, psychosocial, obstetric, and gynecologic history and record findings in a systematic, accurate, and succinct form. Perform a complete physical examination, with specific emphasis on the reproductive system. Manage post-operative care, performing clinical follow-ups and collaborating with surgeons and healthcare team. Order, perform, and interpret routine laboratory tests. Arrange referrals to other members of the health-care team as needed and to community resources as appropriate. Monitor and make recommendations to nursing policy and procedure manual to include development of nursing protocols and procedures for same. Partner with the Division Director, Department Administrator and Practice Manager on quality standards, meaningful use, clinical workflows, nursing education and the delivery of exceptional patient care. Perform inpatient post-operative rounds, consultations and emergency department consultations as necessary. Performs other duties as assigned. Overview Function as an independent nurse clinician in coordinating patients to include the performance, interpretation, and reporting of test results, assessment of patient status, educational needs, and follow up care. Provides education for staff and community and assists with research programs. Responsibilities Recognize normal versus abnormal results; prepares accurate, readable report of the test; contact physician when necessary. Assess health education needs of patient to provide ongoing evaluation of necessary instructions. Provide a holistic approach in the care of all patients. Triage incoming patients by obtaining vital signs, reconcile current medication list, reconcile allergies and determine chief complaint. Administer injections of medications or immunizations in accordance with approved nursing protocol or standing orders and document appropriately in the patients’ electronic medical record. Observe patient for any adverse reactions. Assist with and maintain ongoing knowledge of billing and insurance protocols. Assist physicians with examinations, minor surgeries, diagnostic procedures and treatments. Implement standing orders when applicable. Provide basic care as ordered. Maintain clinical competency in areas of responsibility to ensure quality patient care which includes validation of skill level by passing both verbal and written tests administered by Supervisor. Promote the philosophy, goals and objectives of the School and the Department by becoming familiar with and complying with policies and procedures. Collects samples, such as urine, blood and sputum, from patients for testing and perform routine laboratory test on samples. Must be able to log in collection sample and follow-up on results. Respond to questions from patients, family members, faculty/residents, other health care providers in a timely and professional manner. Patient phone calls should be answered or documented the same day they are received. All patient conversation must be documented in the chart, as well as, attempts to reach patient. Review and work/complete all “tasks” assigned to you and your individual team (as well as covering for other nurses who may be absent. This will include responding to patient messages, prescription refills, lab results, referrals, home health agencies, medication prior authorizations, patient forms and follow up from physicians. Patient education; diabetic teaching of home monitoring, coagulation therapy, discharge medication reconciliation, drug assistance programs nutrition and barriers to care. Participate in staff meeting, education lectures, and PCMH/QI improvement activities. Sustain the quality assurance of patient charts. Monitors clinical supply levels and inventory for the entire facility and place orders as needed. Performs venipuncture as needed. Completes patient’s FMLA and disability forms. Overview The Department of Medicine has developed a Comprehensive Sickle Cell Program in collaboration with the Departments of Physical Medicine and Rehabilitation and Psychiatry. This program is supported by EVMS and Sentara Health. The goal of this program is to provide top-notch outpatient and inpatient clinical care to patients with sickle cell disease. We are looking for Advanced Practice Providers (APP), a nurse practitioner (NP) or a physician assistant (PA), who will function as a clinical provider in the Division of General Internal Medicine, Department of Medicine. The APP will participate in all aspects of patient care and will participate in the supporting roles of consultant and educator as indicated. The clinical practice will mostly focus on clinical care of patients with Sickle Cell Disease in the hospital and in the outpatient clinic under supervision of clinical faculty. The clinical practice may also include provision of primary care to all types of patients. Responsibilities Independently assess, diagnose, & develop treatment plans for patients with sickle cell disease as well as patients in the area of Primary Care in accordance with protocols and standards of care. Promote a schedule of health promotion examinations and interventions in our patient population Obtain medical histories, perform physical examinations, and prescribe, dispense or deliver medications under a collaborative practice agreement. Overview The Advanced Practice Provider (APP) will play a pivotal role within the EVMS Department of Medicine, Hospitalist Division, with a focus primarily on night shifts. In this dynamic position, APP will take charge of managing health issues and coordinating healthcare in progressive care or acute care in-patient hospital settings. As an integral part of our team, the APP holds the responsibility of delivering top-notch patient care and medical services to individuals, families, and/or groups within the hospital setting. This includes working independently and collaboratively with a sponsoring physician, and internal medicine residency program teaching teams, providing a wide range of health care services. The scope of practice encompasses conducting thorough health assessments, diagnosing medical conditions, developing personalized medical plans of care, and prescribing/providing treatments and interventions. APP will also play a key role in evaluating patient responses, making referrals, and adeptly managing both acute and stable chronic illnesses.
