Position Highlights: The inpatient Advanced Illness Management (AIM) Nurse Practitioner or Physician Assistant will provide palliative care to adults with advanced, life-limiting progressive and terminal illness. Candidate will provide initial and ongoing clinical assessment for patients with advanced and/or progressive medical conditions who are consulted for palliative care. The NP or PA-C will collaborate with the AIM team (social work, chaplain, AIM physician), inpatient attending physician, primary care provider, specialist, patient and their families to effectively manage the palliative care needs of the patient.
