The Village at Willow Crossings is looking for a Full Time RN/LPN to join our team! Schedule: Monday 7a-3p, Thurs/Friday and every other weekend 2p-10p. Coordinates care needs with community providers via an effective case management process. Effectively balances service demands with supporting resident independence. Recognizes and assists in the prevention of elder abuse, neglect, and exploitation (financial and other) and reports to appropriate sources per state regulations. Responds to emergencies and personal emergency response system and knows when to call for backup. Report all resident concerns and unavailable medication while administering the medication to the RCD or Wellness Nurse. Restock medication cart after all medication passes. Assist in checking medication regardless of packaging system. Assess the residents to determine need for “as needed medication” and appropriately document and report to supervisor. Count all narcotics with another Medication Care Manager (MCM) or Lead Care Manager (LCM) each shift and report discrepancies to the RCD or Wellness Nurse. Maintain and clean the Medication Room, medication carts, and treatment carts for neatness, cleanliness, availability of medications, and expired medications. Follow re-fill process for medications. Responds to emergencies and personal emergency response system and knows when to call for backup.
