Overview Under direct supervision of a Registered Nurse or physician, the LPN performs nursing duties in the care and treatment of patients on an assigned unit. Responsibilities Demonstrates proficient knowledge in the treatment of patients requiring nursing care. Uses available diagnostic results and other information to make an accurate evaluation. Applies knowledge of illnesses, injuries, and diseases and uses available resources in performing the nursing process. Performs timely, comprehensive, and accurate reevaluations and documentation at regular intervals as defined by the department, when a significant change occurs in the resident's condition, and when otherwise warranted. Is considerate of residents' privacy during the evaluation process. Contributes to an individualized and appropriate plan of care delineating measurable patient care goals Contributes to the plan of care making necessary changes as the condition of the patient indicates. Bases care decisions on resident needs and care priorities. Monitors patient outcomes. Monitor continued care needs for discharge and coordinates resources to meet post-institutional requirement.
