Assist in checking medication regardless of packaging system. Assess the residents to determine need for “as needed medication” and appropriately document and report to supervisor. Count all narcotics with another Medication Care Manager (MCM) or Lead Care Manager (LCM) each shift and report discrepancies to the RCD or Wellness Nurse. Maintain and clean the Medication Room, medication carts, and treatment carts for neatness, cleanliness, availability of medications, and expired medications. Follow re-fill process for medications. High School Diploma or GED Graduate of an accredited Certificate, Diploma, or Associate School of Nursing Current State License as a Licensed Vocational / Practical Nurse 1 year of experience in a clinical care setting Experience in a hospice or home health environment is preferred 50% travel required Ability to frequently lift, push, pull, and support up to 50-100 pounds, including positioning or transferring patients and moving equipmen Valid driver’s license and insurance and reliable transportation to perform job tasks You will report to the Clinical Team Manager.
