As a result, the company is working to provide our differentiated, quality outcomes to as many patients that would benefit through our out-patient clinic partnerships Under the direction of a Registered Nurse (RN) or Provider, the Wound Care Center Licensed Practical Nurse (LPN) provides wound-related and other basic nursing care to the center’s patients according to all the WCC and hospital policies and procedures, care and safety protocols, and Clinical Services Practice Guidelines. All Healogics employees must perform their job responsibilities according to all Healogics policies, Hospital policies, as well as to accrediting organizations, federal and state regulation, and to the Centers for Medicare and Medicaid Services (CMS) guidelines, as applicable. Essential Functions/Responsibilities: Under the direction of the RN/Provider, provides wound-related patient care according to Basic Life Support (BLS) and approved and hospital protocols, Policies & Procedures Assist in vein clinic as assigned after Healogics Vein Clinic training completed May assist with intake and discharge. May provide assistance with case management under the direction of RN, in accordance with state regulations Gathers patient information. Gathers necessary equipment and supplies for the RN or provider. Reports quality of care issues to Clinical Nurse Manager. Communicates as needed with all team members regarding therapeutic interventions, to ensure quality of care. Performs hyperbaric oxygen therapy related duties, as needed, after completing the required Healogics hyperbaric training May perform hyperbaric safety director duties after completing the required Healogics hyperbaric safety director training May function as a Documentation Assistant (scribe) in accordance with Healogics policy Performs other duties as delegated. Required Education, Experience and Credentials: High School Diploma or General Education Development (GED) Graduate from an accredited practical nursing program/state approved educational program Current Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license in the state of practice -to maintain throughout the duration of employment in the position Two (2) or more years’ experience as LPN/LVN, to include one (1) or more years in SNF or LTACH setting Wound care experience preferred Required Knowledge, Skills and Abilities: Demonstrated competency in BLS (Basic Life Support) Demonstrated organization, prioritization and time-management skills Basic knowledge of regulatory/accrediting requirements for healthcare organizations. Service Job Summary: JOB SUMMARY Supports the licensed healthcare team members in the provision of patient care to include direct patient care, providing healthcare information to patients/families, and following established plan of care. Works in close collaboration with the RN or a licensed healthcare team member to evaluate the patient's plan of care and progress toward goal attainment. All patient care activities are undertaken with the delegation/supervision of a licensed professional in accordance with VUMC standards and licensure regulations. . The person in this position will support and facilitate the workflow of CT and MRI by starting 35 or more peripheral IVs per shift or accessing/de-accessing port-a-caths for the infusion of contrast for exams. SHIFT This position will work three 12-hour shifts per week from 6:00a - 6:00p for a total of 36 hours per week to include some weekends. What you'll need: - Graduate of a discipline specific program - Active LPN license - 1 year of LPN experience. Contributes to the nursing assessment by collecting, reporting, and recording objective and subjective data in an accurate and timely manner. • LPN Patient Education (Novice): Possesses sufficient fundamental proficiency to successfully demonstrate patient education in practical applications of moderate difficulty. Generally works under the direction of others while accomplishing assignments. • LPN Care Planning (Novice): Participates in the development of the plan of care in consultation with the licensed healthcare team. Demonstrates ability to anticipate resource needs, identify distinct tasks, set priorities, schedule activities, meet deadlines, and organize work within areas of direct responsibility. Competently handles multiple assignments often simultaneously, prioritizing work into manageable and measurable units. Ability to adapt to changes to patient care and area needs with minimal disruption and loss of productivity. Follows up to assure problems and issues are resolved. • Clinical Competency (Novice): Possesses sufficient fundamental proficiency to successfully demonstrate nursing interventions (such as wound care, point of care testing, vitals signs, etc.) in practical applications of moderate difficulty.
