Description The after hours nurse supports our families needs after hours. After hours is qualified as after 5PM and weekends. The professional nurse is accountable to the Director of Patient Care Services for implementing the patient plan of care and communicating each patient needs to the Interdisciplinary team. The professional nurse is also responsible for communication and collaboration with community physicians as well as family members regarding individual patient care. Requirements Patient Care The Hospice Team Nurse provides professional nursing care to patients by utilizing all elements of nursing process. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order. 
Conduct and coordinate the completion of the resident assessment in accordance with the requirements of this state and policies and goals of the Facility. Conduct and coordinate the completion of the RAI process in accordance with current rules, regulations, and guidelines that govern the resident assessment in accordance with Medicare, Medicaid, OBRA, and other payer programs. Develop, implement, and maintain an ongoing quality assurance program for the resident assessment. Participate in Facility surveys (inspections) made by authorized government agencies. Monitor the Facility’s QI and QM reports to ensure that appropriate corrective action can be implemented when potential problem areas occur. Provide initial and ongoing education with the interdisciplinary team involved in the RAI process. Experience required: Valid LPN or RN License in PA 1 year or more of ICD-10-CM Coding preferred 1 year or more of SNF insurance reviewing preferred Townview currently has a FT opening for our afternoon/midnight Weekend Program.
