Implement all available actions to prevent avoidable hospitalizations and ER visits. Provides positive, supportive communication to physicians, patients, families, visitors, and agency personnel. What is Required? High School Diploma or GED Graduate of an accredited Certificate, Diploma, or Associate School of Nursing Current State License as a Licensed Vocational / Practical Nurse 1 year of experience in a clinical care setting hospice preferred Experience in a home health environment is preferred 50% travel required Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
