The Licensed Practical Nurse's primary role is to provide nursing care for patients who are experiencing common, well-defined health problems. Accurately and thoroughly documents all findings in the patient record and identifies expected outcomes based on assessment data; effectively communicates pertinent assessment findings with other health care members. Implements nursing care interventions identified in the individualized multidisciplinary treatment plan using accepted standards of practice in collaboration with other health care providers; performs nursing procedures/treatments as prescribed by physician/LIP. Conducts specified bedside laboratory tests, wound care, and provides for comfort and assistance in the performance of ADLs; Promotes the patient's ability to cope, adapt and/or solve situations related to illness and/or stressors. Provides supportive, age appropriate communication and culturally acceptable interventions and documents special needs. Provides for the safe delivery of medications in accordance with the facility policies and physician orders; administers medications utilizing established safety procedures / standards. Ensures patients are effectively assessed prior to and after medication administration. Accurately communicates/documents medication doses prescribed, and provides patient education on medication administered. Follows up on patient care responsibilities; communicates incomplete assignments and patient needs with other disciplines to assure continuity of care; transcribes orders accurately and performs chart audits of transcription accurately and according to facility guidelines. Utilizes nursing knowledge to successfully meet the needs of the facility, patient population and staff members. Utilizes nursing resources to enhance evidence-based practice. Guides, educates, and provides leadership as a preceptor to new staff. Performs other duties and projects as assigned.

Perform routine assessments to monitor patient health and ensure overall well-being. Manage patient access to healthcare and connect individuals with community resources for medication administration, self-care techniques, and disease management. Conduct medication reviews to identify potential side effects and drug interactions. Develop and administer mental health assessments to screen for depression and evaluate mood, behavior, and cognitive function. Accurately document patient medical history—including illnesses, medications, allergies, surgeries, and family history—using electronic health records. Use your excellent communication skills to interact with patients, families and other healthcare professionals To be considered for this important opportunity to care for patients in the community, you must meet the following qualifications: Active and in good standing state LVN or LPN license. Successful completion of the NCLEX-PN (National Council Licensure Examination for Practical Nurses). Knowledge of medical terminology and clinical documentation standards. Experience with phlebotomy, vaccine administration, and point-of-care testing. Excellent communication and organizational skills. Experience working in a clinical setting with diverse cultural dynamics among staff and patients. Proven ability to manage sensitive information ethically and responsibly. Strong problem-solving, critical thinking, and decision-making skills. Proficiency with electronic medical record (EMR) systems; Epic experience preferred. Educational Qualifications: High school diploma or GED Graduate of a State approved Practical or Vocational nursing program. Pay Range The typical pay range for this role is: $22.40 - $48.67 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. Responsibilities •Complete and scan required record keeping forms/charts upon the guest’s admission, transfer, and/or discharge.•Chart nurses’ notes in an informative and descriptive manner that reflects the care provided to the guest, as well as the guest’s response to the care.•Report all discrepancies noted concerning physician’s orders, diet change, charting error, etc., to the Director of Nursing.•Ensure that appropriate documentation concerning unauthorized discharges is entered in the guest’s medical record in accordance with established procedures. Qualifications •Current, unencumbered LPN/LVN license required or Compact State license- required.•A minimum of 2 years LPN/LVN experience required.•Able to educate patients and family on multiple disease processes and have good medication knowledge and wound care experience.
