We are looking for a compassionate and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the Union/Florence, KY area to administer patient care in accordance with a physician established care plan. This role is a 1099 contract opportunity, with care being provided at the patient’s residence. Patients range from weekly hours to 24-hour care. This allows for multiple scheduling options, from short visits on a weekly basis to full 12 hour shifts or a blend of both. Patients range from high acuity to those who only need basic medical management. Once our patients are accepted into the program, there is no expiration for this benefit, which allows our team of caregivers to build lasting relationships with their patients for years to come. You’ll get to advocate for the patients and work one-on-one to provide them with the care that they need. This role will collaborate with CNA’s, HHA’s, Case Managers, and the patient’s physicians. Shifts: Multiple shift options available. 8 hours per month with potential for more. Perfect for someone looking for PRN or Part time. Offers are contingent upon passing a background check. #INDLIC \n Responsibilities Execute physician prescribed plans of care. Complete compliant documentation of the care provided in the system of record. Manage and administer prescribed medication, treatment, and therapies. Conduct patient assessments, coordination of care Perform various activities associated with daily living. Monitor vitals and GI intake and output. Assess skin integrity and administer wound care. Monitor for changes in the patient’s condition such as weight loss/gain, self-care abilities, and indicators of disease progression. Communicate with family members, physicians, case managers, and other prescribed individuals regarding the health of the patients. Provide education and training to the patient, their family, and/or the Home Health Aide. Work with patients to be done one-on-one in the patients home to provide for a comfortable environment. Collaborate with and supervise Certified Nursing Assistants and Personal Care Attendants. Perform other personal care services as necessary to meet the patient’s needs. Minimum Qualifications Education, licensing, and certification: graduate of an accredited college or university, Associate or Baccalaureate School of Nursing Current State or Compact License as a Registered Nurse or Licensed Practical Nurse. Current CPR certification TB test and physical examination Maintain required insurance. Good interpersonal skills Hand-eye coordination and manual dexterity Ability to stand or walk for prolonged periods, with the ability to lift up to 50 lbs and move patients. Ability to operate under stressful conditions and make quick decisions. Must have dependable transportation and be willing to commute to the patient’s home. Ability to provide basic medical management up to medically fragile end of life care. Preferred Qualifications minimum of 1 year experience as an RN/LPN in a clinical or home health setting. Ability to assist with two person assist \n $35 - $50 an hour RN: Up to $50/hour, based on years of experience. Job Description Enables the physician to provide quality patient care, and assists in examination and treatment of patients under direction of physician by performing the following duties. Responsibilities Maintains patient flow during clinic hours. Documents review of systems; measures vital signs such as pulse rate, respirations, blood pressure, weight and height (see Appendix A for specifics); updates past and current medical, social and family history; abstracts new procedures; reviews medications and updates medication list at each visit. Correctly records patient information in the electronic medical record (EMR). Assessment is an ongoing process that consists of participation with the registered nurse in the determination of nurse care needs based upon collection and interpretation of data relevant to the health status of a client. Prepares patient charts in EMR for visits by scanning, abstracting, and reviewing to make certain that all diagnostic test results, hospital reports and other medical records necessary to help the physician care for the patient are in the patients chart. Accesses and navigates websites or calls outside facilities to obtain necessary information. Refills medications per MD’s orders and pre-authorizes medications including communicating with the patient, pharmacy, or insurance company as needed to complete the task. Maintains pharmacy cabinet and recording of patient samples. Completes patient assistance paperwork/requests as needed. Preforms basic triage per specific office protocol procedures. Returns calls as needed. Responds to MyChart as needed. Works in-baskets, daily work flow logs, and work queues in EMR/Practice Management system. Answers incoming calls, schedules appointments, checks-in and out patients, and completes pre-certifications for prescriptions, procedures, and surgeries as needed. Operates EKG, cardiac monitors, and other equipment to administer routine diagnostic tests as needed and other clinical duties such as phlebotomy and cast removal as assigned, (see Appendix A for specifics) as well as injections of controlled substances that only a LPN or RN is licensed to administer. Maintains safe, secure, and healthy work environment by following and enforcing standards and procedures; complying with legal regulations. Maintains patient confidence and protect operations by keeping patient care information confidential. Understands and adheres to the legal responsibilities and requirements with the medical assistant role. Prepares treatment rooms for examination of patients; maintains clean, fully supplied exam rooms. Ensures proper infection control, exam room cleaning is performed daily. Orders medical supplies as needed. Keeps an inventory and orders all clinical supplies. Provides explanation and education to patient. May manage phone triage of patient concerns as applicable. EDUCATION: Graduate of an accredited school of Practical nursing. Must currently be licensed in the State of Ohio. YEARS OF EXPERIENCE: 1-3 years LPN experience required, physician office experience required. LICENSES & CERTIFICATIONS: The Licensed Practical Nurse (LPN) is a graduate of an accredited School of Nursing, who holds a current license to practice nursing in her/his state. Membership in professional organizations is desirable. BLS certification required.
