A dynamic professional clinician focused on population management with demonstrated capability to provide relationship-based care within a collaborative multidisciplinary team. This registered nurse utilizes advanced critical thinking to develop, implement, and evaluate an individualized plan of care that educates, empowers and optimizes patient outcomes throughout all transitions within the continuum of care. In addition, this position requires clinician provide afterhours/weekend coverage to serve as primary point of contact for pre and post-transplant patients and internal and referring providers. Business and after hours care coordinator responsibilities include telephone assessment using strong clinical knowledge base, complex care coordination and treatment based on comprehensive assessment and strong interpersonal communication skills, calling in patients for transplant, and ensuring all multidisciplinary members of the transplant team are kept informed of clinical decisions and patient response to interventions in a timely manner. Job Summary We are seeking reliable and dedicated caregivers to be on call for 24-hour shifts. As an On-Call Caregiver, you will be available to fill in for any last-minute shift call-offs if a replacement cannot be found. You will receive a $50 stipend for each 24-hour on-call period, plus your standard hourly pay for any hours worked during a call-in. Assist in checking medication regardless of packaging system. Assess the residents to determine need for “as needed medication” and appropriately document and report to supervisor. Count all narcotics with another Medication Care Manager (MCM) or Lead Care Manager (LCM) each shift and report discrepancies to the RCD or Wellness Nurse. Maintain and clean the Medication Room, medication carts, and treatment carts for neatness, cleanliness, availability of medications, and expired medications. Follow re-fill process for medications. Medical Assistant Trainee (CMA) - Earn While You Learn Program Performs basic clinical skills, as delegated by licensed medical professionals and in accordance with policy and procedure, to promote patient health and wellness. Identifies patients prior to assisting with any intervention. Assists with the collection of patient vital signs and weights and documents accordingly. Assists with the patient rooming process and documents in the electronic medical record including setting specific patient screenings. Promptly reports abnormal findings and changes in patient condition to the registered nurse, licensed independent professional, or preceptor. Assists with patient mobility using safe lifting equipment and following mobility standard operating procedures for safe transfer and positioning practices. Assists providers and clinical team with general patient care, including assisting with physical assessments, examinations, and procedures. Assists with preparing examination/procedure room. Assists with preparing and maintaining sterile fields. Assists with patient procedure preparation. Assists with obtaining labels and sending/transporting specimens for diagnostic testing. Cleans equipment and performs safety checks as needed and per protocol.
