Documents patient information in the medical record to include update vital signs, medications, test results, and patient concerns, etc. Maintains clinical and administrative knowledge base and designated skill set in order to meet established goals for daily call metrics, and defined individual/team goals. Understands insurance restrictions for durable medical equipment (DME) and supply ordering, lab work, referral needs, etc. Assists in the ordering or processing of DME and supplies and coordinates the scheduling of services (e.g. mobile phlebotomy) or referral needs on behalf of the clinical care team. Ensures care coordination and complete communication (closes the loop and follow up) between internal and external individuals, in an appropriate and timely way.
