Responsible for directing the care and services to residents in the Skilled Nursing Facility. Develops, coordinates and manages nursing programs and activities to meet the needs of the residents and promote resident/family satisfaction. Works with staff and other departments to create a work environment which enables employees to carry out their assigned job duties in a safe, effective manner, and which promotes employee satisfaction. Carries out job duties in a manner that supports Hill View’s mission and purpose. ESSENTIAL DUTIES AND RESPONSIBLITIES: Maintains knowledge of and implements systems that assure compliance with all state and federal laws/regulations for long-term care. Oversees the development, implementation, and evaluation of daily clinical operations, to assure effective resident care/services, compliance with federal, state and local regulations, and effective collaboration between all members of the health care team. Makes daily rounds of the entire unit, to oversee resident care and staff performance. Communicates daily with Assistant Directors of Nursing about key operational, resident/family satisfaction, and staff performance issues. Interacts and communicates effectively with all members of the interdisciplinary team including residents, family members, staff, contractors, and physicians. Participates in facility surveys/inspections. Communicates effectively with surveyors/inspectors. Communicates with Assistant Directors of Nursing about resident/family complaints and missing item reports, and their investigation. Works with Assistant Directors of Nursing to development and implementation of a corrective action plan. Oversees follow-up with resident/family members and appropriate staff, and related documentation. Obtains written and oral reports from the Assistant Directors of Nursing about clinical operations within assigned neighborhood and areas of responsibility. Periodically monitors resident documentation to assure that required documentation is present and accurate. Coordinates and oversees new employee orientation to assure that the orientation is being completed in an effective manner, following the structured orientation plan, including skills checklists. Solicits feedback from the new employee, and Assistant Directors of Nursing to evaluate the new employee’s individual needs and the effectiveness of orientation. Provides regular, ongoing coaching to each new employee. Oversees completion of skills competency checks and associated documentation for all RNs, LPNs, and STNAs on an annual basis. Assures that orientation and skills checks are documented and forwarded to Human Resources for placement in staff personnel files. Monitors staff performance on all shifts through direct observations and feedback from Assistant Directors of Nursing. Provides regular on-going coaching to each employee related to individual performance. Assesses individual employee and group training needs for all Health Care Unit staff. Develops and oversees implementation of training programs on clinical issues and operational needs which address learning needs and comply with regulatory requirements. Assures that records of staff training are maintained. Provides leadership coaching and training that includes the administrative and supervisory principles essential for Nurse Supervisors/Charge Nurses. Encourages and supports leadership abilities of staff at all levels. Addresses employee performance issues in accordance with established policies. Conducts employee performance evaluations. Responsible for development and maintenance of written department-specific policies and procedures and resident care programs. Develops methods for coordination of nursing services with other resident services to ensure the continuity of the residents’ total regimen of care. Develops, implements, and maintains an ongoing Quality Improvement program for the Skilled Nursing Facility. Coordinates Quality Improvement activities, including scheduling and leading the Quality Improvement Committee. Periodically reviews nurses’ notes to determine if the care plan is being followed. Assists the Pharmacy Consultant in developing, maintaining, implementing, and updating policies and procedures for the administration, storage, and control of medications and supplies. Participates in Care Plan meetings as necessary. Oversees staff schedules that comply with state staffing requirements and Hill View policies/procedures, and meet the staffing needs of the department. Participates in the recruitment and selection of nursing staff. Conducts interviews in accordance to established processes. Processes employee payroll in accordance with established policies/procedures. Works with the Human Resources Department to ensure that all RNs and LPNs have current licensure in the state of Ohio. Maintains a listing of all STNAs in the Ohio Nurse Aide Registry information. Communicates with the State Nurse Aide Registry about mandated issues in Ohio Department of Health regulations. Conducts staff meetings at least once monthly. Documents via attendance records and meeting minutes. Participates in the interviewing and selection of residents for admission to the facility. Provides the Social Services, Admissions Director and Administrator with information about the Health Care Unit’s ability to meet each potential resident’s health care needs. Assists Assistant Directors of Nursing with physician rounds as necessary. Monitors physician visit frequency, progress notes, and orders for compliance with rules and regulations, and follows-up on problems. Provides direct resident/nursing care as necessary, to ensure that resident needs are being met in a timely manner. Develops inventory maintenance levels for department supplies and equipment, including personal protective equipment, medications, medical supplies, mechanical lifts, gait belts, etc. Periodically monitors inventory to assure that supplies are being maintained. Oversees fire safety and disaster preparedness drills for the department. Ensures that drills are conducted in accordance to established standards. Maintains records of fire and disaster drills. Oversees staff use of safety equipment and supplies (mechanical lifts, etc.) as indicated on the plans of care. Works with the Maintenance Supervisor to develop a preventive maintenance program for unit equipment, to ensure that equipment is safe to use and in good operating condition. Responsible for the financial management of the Skilled Nursing Facility through efficient scheduling practices, monitoring of overtime, utilization of temporary agency staff, and monitoring of supply/equipment utilization. Reviews complaints and grievances made by the resident. Assists Assistant Directors of Nursing in addressing resident/family concerns/issues as needed. Develops a written report, which defines what action was taken to resolve the complaint or grievance. Follows facility procedures and regulatory requirements in addressing/documenting resident complaints. Reports and investigates all allegations of resident abuse and/or misappropriation of resident property in accordance with Ohio Department of Health rules. Provides regular feedback to the Administrator about issues related to areas of responsibility. Makes written reports/recommendations concerns operations of the Health Care units as needed. Maintains a reference library of written nursing material that will assist the unit in meeting the day-to-day needs of the resident, including current medication reference sources, rules and regulations, and Ohio references. SUPERVISORY RESPONSIBILITIES: Directly supervises Assistant Directors of Nursing, Shift Supervisors and Unit Clerk. Directs staff in the Skilled Nursing Facility. Carries out management responsibilities in accordance with the organization’s policies and applicable laws. Responsibilities include interviewing, hiring and training employees, planning, assigning and directing work, appraising performance, rewarding and disciplining. QUALIFICATIONS: Current licensure as a Registered Nurse in the state of Ohio required. Strong leadership, interpersonal and management skills required. EDUCATION AND/OR EXPERIENCE: Associate Degree in Nursing required. Bachelor’s Degree in Nursing preferred. Minimum three years clinical nursing experience required. Previous long term care and management experience preferred. LANGUAGE SKILLS: Must have ability to read, analyze and interpret common health care related and technical journals, financial reports, legal documents and government regulations. Ability to respond to common inquiries or complaints from customers, regulatory agencies, or members of the business community. Ability to effectively present information to individuals and small groups including residents, employees, top management staff. MATHEMATICAL SKILLS: Must have ability to add, subtract, multiply, and divide all units of measure using whole numbers, common fractions and decimals. Ability to read and interpret financial reports. Ability to compute rate, ratio, percentages, and to interpret graphs. REASONING ABILITY: Must have the ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with abstract and concrete variables. PHYSICAL REQUIREMENTS: Frequently required to stand, sit, walk, speak, and listen. The employee is occasionally required to use hands and arms to feel and reach. Monitors I.V. infusions at least once hourly for the following: type of fluid, infusion rate, position, and site appearance. Problems with I.V. site are addressed immediately by an RN. LPNs will report problem to an RN immediately. IV medications are delivered by RNs. When no RN is on duty, LPN will verify that RN is available for next medication dose. Performs a head to toe assessment/evaluation of Medicare skilled residents each shift, including vital signs. Documents this assessments. Documents episodic events, such as resident falls, response to antibiotic therapy, changes in condition, etc. in the resident’s nurse’s notes/Point Click Care system. Makes rounds with physicians and communicates with physicians about changes in resident’s conditions in the absence of the Charge Nurse/Supervisor. Assures that physician orders/progress notes are documented on the appropriate form and signed. Takes telephone orders from physicians and records on the Physicians’ Order Form. Notifies family members when significant events or changes in resident’s conditions. Makes and modifies assignments for Nurse Technicians to best meet resident needs. Supervises care delivery by Nurse Technicians, including observation of care, providing direction, and having regular on-going communication, to ensure that direct care is being given according to each resident’s Plan of Care and established standards. Assists Nurse Technicians with direct care, including answering call lights, feeding, and general care when resident needs are high. Uses Point Click Care system for documentation of behavior and other information as needed. Assists or gives bathes/showers. Obtains resident weights as assigned. Organizes, transports, and assists residents to/with activities and therapy programs. Assists with resident admissions including: - greets resident and family member. - orients resident to room number and location, call light system, and layout of room and facility. - obtains vital signs, height, weight and documents these. - unpacks resident clothing and personal items. Assures that clothing and personal possessions are labeled and documented on personal property inventory sheet. - assures that resident has needed personal care items. Understands the general use and cleaning procedures of equipment such as oxygen concentrators. Assures that signs are posted on the door of any room where oxygen is being used. Notify nurse if humidifier needs re-filled. Store oxygen cannula properly when not in use. Follows established infection control policies/procedures. Uses universal precautions when carrying out job duties. Makes resident beds. Changes bed linens at bath time and as needed. Assists residents in 24-hour reality orientation. Runs errands, directs visitors and answers telephones as needed. Follows safety policies/procedures for preventing, identifying, correcting and reporting hazards and incidents.
