Location: Butte/Dillon Montana (Hybrid In-home/remote) Position Summary: The Clinical Manager establishes and implements Granite Peak Home Health Skilled Home Care Processes. The Clinical Manager or qualified designee provides oversight of all patient care services and personnel, including making patient and personnel assignments, coordinating patient care, coordinating referrals, ensuring that patient needs are continually assessed, and ensuring the development, implementation, and updates of the individualized plan of care. Qualifications: Minimum of 2 years of nursing experience in Home Health (preferred). Associate’s degree in nursing (required); bachelor’s in nursing (preferred). Acceptance of the agency’s philosophy and goals. Ability to exercise initiative and independent judgment. Management experience and knowledge of Montana Home Care License, CMS Skilled Home Care License, and federal, state, and insurance reimbursement programs. Basic computer skills and proficiency with web-based software and electronic health records (Wellsky). Must pass a criminal background check by the Department of Human Services and not be on the OIG exclusions list. Must have a cellular phone for work-related use. Principal Responsibilities: Reporting: Reports to the Home Health Director. Oversight: Provides oversight of all patient care services and personnel, directing the clinical activities of the branch. Clinical Supervision: Provides direct oversight of assessment, planning, implementation, and evaluation of patient and family/caregiver care by all clinical personnel. Staff Management: Leads, manages, and holds Granite Peak Home Health clinical staff accountable for compliance with regulations, policies, and procedures, ensuring excellent, quality, and timely care. Meetings: Schedules and conducts weekly IDT meetings with assigned staff, following processes. Policy Development: Develops and implements processes, policies, and procedures, coordinating and overseeing all patient services provided by clinical personnel. Budget Assistance: Assists the Executive Director Director in preparing and administering the Granite Peak Home Health budget. Operational Monitoring: Interprets operational indicators to monitor census changes and adjusts staffing levels, revenues, or expenses accordingly. Compliance: Ensures compliance with the Granite Peak Home Health Employee Handbook and policies. HR Functions: Participate in the hiring, evaluation, and termination of organization personnel. Orientation and Training: Develops, implements, and evaluates orientation for new Granite Peak Home Health personnel and plans in-service and continuing education programs. Performance Evaluation: Conducts quarterly conversations and annual performance evaluations of supervised staff. Record Maintenance: Collaborates to maintain accurate patient records and monitors statistics and reports for evaluation and reporting. Supply Management: Ensures adequate and appropriate supplies and equipment for patient services. Quality Assurance: Assists with evaluating performance via the QAPI program, ensuring staff productivity and safe, quality home health services. Goal Development: Participates in developing goals and administers Granite Peak Home Health policies and procedures. Regulatory Compliance: Collaborates with the HR department to ensure compliance with local, state, and federal laws regarding licensure and certification. Industry Awareness: Stays informed about nursing and home health care changes, adjusting procedures accordingly. Regulatory Knowledge: Remains updated and familiar with the rules and regulations of third-party agencies, state licensure, and Medicare standards. Referral Promotion: Promotes referrals to Granite Peak Home Health through community awareness, marketing plans, and building relationships within the communities Granite Peak Home Health serves. Staff Development: Identifies training needs and opportunities for professional development among staff. Creates and implements development plans to enhance staff skills and competencies. Mentors and supports staff in achieving their professional goals. Organizes workshops, seminars, and other educational activities to foster continuous learning. Encourages a culture of continuous improvement and professional growth within the team. Performance Metrics: Establishes and monitors key performance indicators (KPIs) for clinical staff to ensure high-quality patient care. Tracks and analyzes patient outcomes, satisfaction scores, and other relevant metrics to identify areas for improvement. Reports on performance metrics to senior management and uses data to drive decision-making and process improvements. Ensures compliance with all regulatory and accreditation standards through regular audits and reviews. Under general supervision, utilizes the nursing process of assessment, planning, implementation and evaluation, and theoretical/clinical skills in the promotion/maintenance of health. Incorporates evidence- based nursing practices consistent with national standards and which support quality outcomes. Assumes responsibility and accountability for the quality of nursing care rendered. Essential Job Functions Supports and models behaviors consistent with Billings Clinic’s mission, vision, values, code of business conduct and service expectations. Utilizing a holistic approach and evidence based nursing practices, incorporates the nursing process into the goal directed plan of care for the patient. Plan of care should include planning and scheduling of intervention/care and collaboration with other services/departments. Prepares and administers various medications and injections; collects and analyzes specimens as directed. Provides and/or assists with patient care, minor procedures and treatments within the scope of practice. Communicates appointments, specialty procedures, and instructions by phone, in writing, and in person; coordinates patient information with other facilities, departments, and staff as appropriate. Provides education, instructions, and advice to patients as directed by physician with consideration to age appropriate care. Responsible for documentation within clinical records (both electronic or written) of all patient care or treatments provided Enters physician orders into clinical information systems. Evaluates patient status via phone and in office setting. Advises patient appropriately (i.e., wound status, post-operative, etc.). Plans and organizes the work day to meet the need of the designated area. Provides office clerical and administrative services as directed or as needed to maximize workflow, patient through put and to maximize provider resources. Assists with provider documentation, charge capture and billing code capture. Delegates work assignments within the scope of practice to other team members (LPN, Medical Assistant or Patient Service Representative) to meet the needs of the facility. Plans, organizes, implements, evaluates and documents safe patient care consistent with organizational values/goals, policies and procedures. Adheres to patient safety standards and identifies and anticipates problems and safety issues and initiates appropriate action. Maintains appropriate skill/knowledge in specialty area with application and patient relevance of commonly used equipment specific to area; pharmacological application; application of laboratory values with disease process and physiological status. Analyzes and shares the practice implications of nursing research in specialty area. Performs or assists with various procedures as competency and training permit (e.g., dobutamine stress tests, conscious sedation, spirometry, IVs, and patient recovery, etc.) Utilizes performance improvement principles to assess, enhance, and/or improve the quality of care. Participates in the incorporation of evidence based outcomes and core principles, state survey/accreditation readiness activities, quality audits, quality improvement and patient safety teams, peer review processes, etc. Develops collaborative relationships with physicians and other health care providers to enhance quality practice. • Takes responsibility for identification of own professional growth and developmental needs; meets all system/departmental educational requirements.
