Responsible for assessment of health needs and contributing to the plan for care for individuals and families. Responsible for the initial implementation of nursing care plans, education of paraprofessional staff, and provision of nursing care as well as the periodic reassessment of individual and family nursing needs. Main Function: Under the general supervision of Obstetrician/Gynecologist(s), provides clinical medicine services and applicable administrative services to Thompson Hospital OB/GYN patients in our beautiful Canandaigua Medical Group building in the heart of Canandaigua, NY. Provides consistent, high quality care consistent with accepted national and/or community standards. Collaborates with office staff in development and performance of efficient office practices. Assures regulatory compliance. Provides patient management and role modeling that unifies department goals with health system goals. Acts in the best interest of the patient. SUMMARY The RN Case Manager is responsible for the delivery of comprehensive nursing care to a set of assigned patients at a specific point in time. This involves the assessment of patient and family needs and the development, implementation and evaluation of an appropriate Plan of Care, making changes in response to changing patient needs. The RN Case Manager identifies appropriate interdisciplinary services needed, coordinates those services and supervises Private Duty Nurses (PDN) as applicable.
