Night (United States of America) Registered Nurse- Clinical Decision Unit The Registered Nurse is responsible and accountable for assessing, planning, implementing and evaluating as per the nursing process. The Registered Nurse is expected to direct, delegate, and coordinate all nursing care provided by the LPN, CNA, Patient Care Companion, and Unit Clerk consistent with their roles, responsibilities, and skill level. Overview The Licensed Practical Nurse will focus on providing high level patient-centered care in a physician office setting by measuring and recording vital signs, placing patients in exam rooms, preparing patients for exams/procedures, assisting the health care provider with exams/procedures, providing patient support during exams, maintaining medical supply inventory, and stocking exam rooms. The LPN will also provide patient care by administering medications, processing patient phone calls per practice protocols, instructing patients regarding health care and treatments, and providing health care provider with information pertaining to patients health status. May be required to float to other UFHP practices, as needed, for staffing. Qualifications Minimum Education and Experience Requirements: Licensed LPN eligible to practice in the State of Florida. The Nurse Case Manager is responsible for independent management of the Home Health patient population requiring the use of advanced assessment, teaching and decision-making skills. The nurse is responsible for ensuring that appropriate referrals to other services are made, interdisciplinary conferencing takes place regularly, and appropriate documentation is completed. Relevant knowledge and experience is consistently applied to new patient populations. The Nurse Case Manager cares for a caseload of home health patients by evaluating the patient for appropriateness of home health and developing the home care plan in conjunction with the physician.
